PRIMARY FUNCTION : Responsible for the provision of evidence - based medication therapy management with a special focus on antithrombotic - related care transitions services and high risk patient populations to optimize patient care and reduce likelihood of antithrombotic - associated complications and re-hospitalizations. Main clinical program efforts will be concentrated at Jefferson Center City, with some collaborative stewardship activities with clinical staff at additional sites within the Enterprise, as needed.

ESSENTIAL FUNCTIONS:

¿ Assures appropriate evidence-based, patient-centered medication plans of care to high risk patient populations in order to assure safe transitions of care, reduce antithrombotic and antiplatelet - associated complication, adverse events and re-hospitalizations.

¿ P rovides direct patient-centered care and integrated pharmacy operational services in a specialized, vascular medicine practice setting as part of a multi-disciplinary team.

¿ Medication therapy management services include, but are not limited to: assessing patient needs, incorporating age and vascular disease specific characteristics into drug therapy plans and patient education, selecting cost - effective therapeutic plans, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication - related adverse events.

¿ Delivers direct patient care and clinical practice, through collaborative practice agreements and service-based programs.

¿ Develops processes to identify and manage high-risk patient populations and implements quality and safety measures to improve medication use and clinical outcomes.

¿ Designs and implements anticoagulation stewardship activities and quality assurance / medication utilization surveillance programs designed to assure optimal and standardized antithrombotic management throughout the institution and Enterprise.

¿ Conducts enterprise-wide clinical research and practice advancement projects, as well as patient care quality and regulatory compliance initiatives designed to improve cardiovascular medication-use processes or pharmacy practices.

¿ Provides medication and practice-related education/training to clinical staff and actively serves as a preceptor for Doctor of Pharmacy students, Doctor of Medicine students and PGY1 & PGY2 pharmacy residents.

¿ Assures pharmacy services is an integral part of the transition of care and outpatient care model and facilitates enhancement and expansion of pharmacy services/programs.

¿ Works with the Pharmacy and Vascular Medicine Directors on Enterprise-wide quality improvement initiatives with the goal of improving services via program implementation, benchmarking, monitoring and analysis to determine program effectiveness.

¿ Tracks and evaluates assigned clinical programs for operational, quality, regulatory and financial efficiency and routinely benchmarks against local and national best practices

¿ Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.

¿ Attend rounds with Vascular Surgery and Vascular Medicine as requested.

Qualifications

EDUCATIONAL/TRAINING REQUIREMENTS :

Doctor of Pharmacy degree

ASHP accredited PGY1 residency

Preferred

ASHP accredited PGY2 Residency

NCBAP Certification as Certified Anticoagulation Care Provider (CACP) or alternative BCPS or other Certification from the Board of Pharmaceutical Specialties with antithrombotic management experience

CERTIFICATES, LICENSES, AND REGISTRATION :

PA Pharmacy License

EXPERIENCE REQUIREMENTS :
  • Knowledge of contemporary hospital/ outpatient clinical practice and service
  • Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in area appropriate for clinical assignments
  • Clinical pharmacy experience (2-4 years) in a hospital setting preferred (required if no PGY2 residency)
