Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

Duties include but are not Limited to:

Anticoagulation Clinic Support:

1. Obtains necessary supplies from laboratory service and stocks, restocks supply checking for proper storage and outdates on test strips.

2. Performs Point of Care (POC) testing for International Normalized Ratio (INR).

3. Reviews medication history, changes (including herbals, dietary supplements, and OTC's), adherence, and complications of therapy and report findings to CPP. 1

4. Accesses patient compliance and attains renewals for Direct Oral Anticoagulants (DOACs) by monitoring the DOAC dashboard.

5. Assist with scheduling future lab orders and follow up appointments.

6. Prints daily schedules and "no shows/lost to follow up" reports. Review both reports and patients' electronic health record (EHR) to identify which patients have and have not been followed up within the last 60 days.

7. Telephone support involves rescheduling patients who have missed appointments, phone interviews of patients who have obtained INRs from outside labs, and triaging incoming phone calls from patients, their family members or other health providers.

8. Reviews all encounters daily and notifies CPP for completion.

Medication Management:

1. Interviews patients, family members and/or caregivers, to develop an accurate list of medications, drug strengths, dosages, routes, and instructions the patient has been taking.

2. Obtains relevant patient information from patient interviews using joint legacy viewer (JLV) with our department of defense (DOD) partners.

3. Compares the compiled list with the active medication profile in the computerized medical record of the patient.

4. Documents the compiled list in the electronic health record, noting any discrepancies or omissions within 24 hours of patient encounter.

5. Updates and clarifies allergy information in patient's records in an electronic health record (EHR).

6. Improves medication safety by collecting up-to-date and prior-to admission medication lists for patients admitted to the hospital.

Hospital Discharges:

1. Prepares discharge medications for prescription/order (s) are reviewed by a pharmacist for therapeutic propriety, reviews orders for technical accuracy and converts the order, if necessary, so that the item dispensed corresponds to the drug and dose ordered.

2. Reads and interprets prescriptions, obtains proper medication or material in the proper dosage strength and dosage form; determines proper amount and counts, pours, 2 or measures the medications; affixes proper label; and records required data on Pharmacy's copy of prescription order.

3. After a final check by a pharmacist, dispenses and or delivers medication in ambulatory care or discharge situations.

4. Assists decentralized pharmacist and other members of the interdisciplinary health care team in ensuring safe and effective care transitions and patient satisfaction throughout the discharge process.

Patient Aligned Care Team Support:

1. Provides reminder phone calls to patients.

2. Assists staff with scheduling and resolving issues.

3. Educates patients to the use of video conferencing visits, My HealtheVet medication profile view and refill use, secure messaging and other technologies as needed.

4. Assists with the review of Prior Authorization Drug Requests by obtaining all necessary information from VA and Care in the Community providers.

5. Monitors all unsigned notes and notifies CPP for completion.

6. Answers questions from pharmacy and non-pharmacy personnel regarding drug distribution.

Vaccinations:

1. Transport vaccinations and associated supplies to clinic area which may include transporting in a government vehicle for offsite locations.

2. Assist staff with scheduling appointments for vaccinations.

3. Provides reminder phone calls to patients and notes when vaccinations may be due.

4. Promotes harmonious relationships within the work environment in support of staffing needs and Veteran patients.

Other duties:

1. Participates in quality assurance, process improvement activities, and performance monitors.

2. Maintain competence by participating in staff development programs.

3. Provide in-service programs for appropriate clinic personnel and pharmacy technicians as assigned.

4. Successfully complete annual competency certifications based on specific clinical functions/activities assigned.

5. Performs all the duties of a pharmacy technician at the lower grades as assigned and assists in other patient care areas as deemed necessary.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Language Requirement: Applicants must be proficient in spoken and written English in accordance with VA Handbook 5005, part II, chapter 3, section A, paragraph 3j.
  • Certifcation Requirement: The applicant must pass a national certification exam and hold an active national certification through either:(1) Pharmacy Technician Certification Board (PTCB), Certified Pharmacy Technician (CPhT).OR (2) National Healthcareer Association (NHA), Certified Pharmacy Technician (ExCPT)

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: Applicants applying at the GS-08 grade level must have at least one year of experience equivalent to the next lower grade level and demonstrate the KSAs below. Examples of specialized experience include:

  • Works autonomously using independent judgement to perform a full range of duties
  • Mentors and trains lower graded pharmacy technicians in the practice setting.
  • Assists advanced/ specialized pharmacy technicians and participate in performance improvement initiatives.
  • Uses independent judgement to perform the full range of duties in the pharmacy.
  • Functions as staff pharmacy technicians in various locations of the pharmacy, including pharmacy contact centers and other non-traditional pharmacy locations, with minimal oversight.
  • Provide interventions independently with patients and their families/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems.

Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:,

  • Comprehensive knowledge of concepts, principles, methodology and policies in a specialized area or section of pharmacy (such as sterile compounding, controlled substances, automation, clinical pharmacy, contact center, etc.) Note: See Specialized Area.in the Definitions Section.
  • Skill in training and orienting new and existing pharmacy facility employees and students on proper policies and procedures.
  • Ability to evaluate, analyze and coordinate workflow and work activities within a specialized area or section of the pharmacy.
