Description: Job Summary:

Supervises the planning, development, implementation/delivery and evaluation/ improvement of rehabilitation therapy programs/services provided within the Care at Home, home health, palliative, hospice service lines and contracted providers in collaboration with the community agency division. Programs/services include but are not limited to physical therapy, occupational therapy and speech therapy. Accountable for delivery of rehabilitation therapy services/ programs which meet or exceed standards/measures for cost, quality, performance, access, member satisfaction and clinical/functional outcomes. Leads programs that add value; are linked to departmental, organizational and business goals/objectives; conforms to organizational, state, federal and regulatory agency standards/requirements.

Essential Responsibilities:

  • Supervises the planning, development, implementation/delivery, coordination of an integrated continuum of rehabilitation therapy services/programs for members eligible for home health, palliative care, hospice and/or extended care benefits.
  • Designs and utilizes systems, methodologies and processes to monitor, evaluate and continuously improve rehabilitation therapy functional outcomes, services/programs and utilization, cost, clinical, member satisfaction and performance measures/standards.
  • Maintains and enhances staff volume/services to meet identified and anticipated service demands/needs.
  • Coordinates referrals for therapy services with other departments, physicians, agencies.
  • Supervises the performance and competency evaluation process for rehabilitation therapy professional staff; identifies and provides for professional development needs; hires, promotes, fires and disciplines staff; evaluates clinical performance of staff and provides clinical supervision and direction.
  • Manages the development and utilization of admission and discharge criteria and clinical protocols for specific diagnoses, as well as review of treatment plans, assessments, evaluations, modalities and the measurement of functional outcomes.
  • Develops, justifies, and monitors financial, capital, and FTE budgets within care at home rehabilitation department.
  • Oversees the evaluation, modification and improvement of existing programs and outcomes; determines feasibility of new programs to meet anticipated service needs.
  • Oversees compliance with health plan, Medicare, Title 22, TJC and all other pertinent local, state and federal regulatory agency requirements.
  • Ensures the highest quality of care is provided and within established departmental policies and procedures, quality/cost/clinical outcomes and established performance standards.
  • Supervises day-to-day operations and clinical practice standards.
  • Collaborates with patients, physicians, other departments, organizations and agencies to address and resolve patient care, transfer/discharge and program service issues.
  • Acts as a clinical practice expert/resource to staff.
  • Ensures patients receive quality care by reviewing documentation, consulting with staff, reviewing care plans, conducting/participating in multidisciplinary team conferences, and reviewing patient satisfaction reports.
  • Coordinates with SCPMG/KFH/KFHP to provide for the seamless transition of patients from the appropriate setting of care (Inpatient, outpatient, home).
  • Addresses quality concerns, complaints and compliance matters to develop systems and policies/procedures to continuously improve clinical/patient care, performance outcomes and departmental administrative procedures/functions.
  • Conducts minimum quarterly field visits with Home Health staff.
Basic Qualifications:

Experience

  • Minimum five (5) years of clinical rehabilitation therapy experience in an acute, home health, outpatient, or community health setting.

Education

  • Graduation from an accredited physical therapy, occupational therapy or speech language pathology educational program with a Bachelors or Masters Degree.

License, Certification, Registration

  • Speech-Language Pathologist License (California) OR Physical Therapist License (California) OR Occupational Therapist License (California)

Additional Requirements:

  • Demonstrated ability to plan and evaluate rehabilitation therapy programs; knowledge of local, state, federal, Medicare and TJC standards/requirements pertaining to home health rehabilitation therapy services.

  • Strong written and oral communication skills.

  • Must be able to work in a Labor/Management Partnership environment.

Preferred Qualifications:

  • Two (2) years of supervisory or program management experience, or teaching experience within a university setting.

  • Five (5) years of home health experience.

  • Geriatric or comparable certifications via the APTA, AOTA or ASHA professional associations.

  • Doctoral Degree.
