Make a Difference in Transitions: Care Transition Coordinator at Aspire Physical Rehabilitation - Cahaba River
Aspire Physical Rehabilitation, a leading provider of outpatient physical therapy, is seeking a compassionate and organized Care Transition Coordinator to join our team at our Cahaba River location!

In this role, you will play a vital role in ensuring smooth transitions for our patients between healthcare settings. You'll bridge the gap between hospitals, clinics, and homecare, advocating for our patients' needs and coordinating their care plan for optimal recovery.

What You'll Do:
  • Collaborate with discharge planners, case managers, and therapists to develop a seamless care plan for patients transitioning from hospitals, skilled nursing facilities, or home care to Aspire.
  • Educate patients and their families about Aspire's services and answer any questions they may have.
  • Facilitate communication between healthcare providers involved in the patient's care.
  • Schedule appointments, manage referrals, and ensure all necessary paperwork is completed.
  • Track patient progress and outcomes to ensure continuity of care.
  • Maintain a positive and professional demeanor, fostering trust and open communication with patients, families, and healthcare professionals.
What You Bring:
  • Minimum 3 years of sales and/or marketing experience in a healthcare setting, preferably in care coordination.
  • Strong understanding of the healthcare system and different care settings.
  • Excellent organizational and communication skills, both written and verbal.
  • Ability to work independently and manage multiple tasks simultaneously.
  • Compassionate, patient-centered approach with a commitment to quality care.
  • Proficient in Microsoft Office Suite.
