DEPARTMENT DESCRIPTION:
CHA’s Health Improvement Team (HIT) works together with healthcare providers, residents, community based organizations and city leadership to assess health status, determine priorities, build action plans and provide leadership around health issues that impact the community.
Summary:
Responsible for the evaluation and treatment of an assigned caseload of clients, providing a range of psychosocial care. Works at one or more service sites, under social work clinical supervision and administrative supervision at the service site or in the service line.

- Current MA licensure as LCSW (Licensed Clinical Social Worker).
- Work Experience: Minimum of clinical internship training or job experience in clinical human service work with population specific to service site.
