Job Summary :

The Clinical Supervisor will be an important member of the Healthy Neighbor Care Team. Responsibilities include: work to improve patient health by providing social support, advocacy, and resource navigation; provide clinical oversight to the Healthy Neighbor Program and supervise Community Health Workers (CHWs); support patients and reinforce diabetes and hypertension education and work with patients to identify barriers to chronic disease management and work toward solutions.

Position Responsibilities:

Provide programmatic-specific clinical education to the Healthy Neighbor Care Team and patients, such as diabetes and hypertension chronic disease monitoring and management education.

Remotely review and interpret clinical data and triage diabetes & hypertension patients as needed using established care pathways. Identify barriers to chronic disease management and work toward solutions.

Utilize metrics to gain insight and promote better outcomes for patients.

Accompany Community Health Workers for the initial in-person patient visit and subsequent in-person visits as needed.

Establish community partnerships that support Healthy Neighbor.

Identify opportunities to improve Healthy Neighbor and work together with the care team to implement changes.

Manage team of Community Health Workers and support to Care Coordinator. Lead weekly rounds. Provide guidance with caseload management, patient referrals to social resources, as well as education and mentorship around building good relationships with patients.

Serve as a clinical resource for the Healthy Neighbor Care Team & enrolled patients.

Position Qualifications Required:

Required Experience:

At least two years of RN experience. Inpatient, ambulatory, case management, home care, and/or triage experience preferred.

Prior supervisory experience preferred.

Prior experience with community-based patient advocacy, patient engagement, & community resource navigation preferred.

Bi-lingual (Spanish) preferred.

Required Education:

Graduate of an accredited school of Nursing. BSN preferred.

Training / Certification / Licensure:

Current NJ RN License, BLS certification

Certified Diabetes Educator (CDE) preferred. Patient Navigation Certification or training preferred.
