Mental Health Case Management Services assist individuals with serious and persistent mental illnesses. These services include assisting clients with obtaining and maintaining entitlements, linking to needed services, monitoring treatment compliance, coordinating care between service providers, providing supportive counseling, providing education on mental illness and medication, creating service plans, and documenting services provided.

Typical tasks include:
  • Client assessment, treatment planning, and monitoring, including community outreach, help clients maintain treatment compliance.
  • Documentation in the electronic medical record of all services provided, quarterly reviews of client progress, and of active service plans in a timely manner and in a format to meet Medicaid and DBHDS standards.
  • Coordination of client services with the client, families, and service providers and advocacy on behalf of clients.
  • Assist with discharge planning for clients that have been hospitalized including linkage to community resources.
  • Good communication skills and knowledge of a wide variety of community resources; ability to find new resources that can be accessed by clients.

Required Qualifications
VOCATIONAL / EDUCATIONAL REQUIREMENT :
Candidates must have BS in Human Services (psychology, social work, special education, etc.) preferably supplemented by considerable experience in working with persons who experience intellectual/developmental disabilities.A master's degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.

EXPERIENCE REQUIREMENT :
In addition to satisfying the vocational/education standards, this position requires a minimum of two years of related, full-time equivalent experience.

SPECIAL CERTIFICATIONS AND LICENSES :
Requires a valid driver's license and a driving record that is in compliance with City Driving Standards.

Must be a QMHP A and have all the knowledge, skills, and abilities as defined by the Department of Medical Assistance Services ( DMAS ) for providers of case management services.

Completion of DBHDS Case Management Modules Training is required to be completed within 30 days after hire.

SPECIAL REQUIREMENT :
Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.

Preferred Qualifications
Bachelor's degree and at least three years of experience providing services to mentally ill clients in the social work, counseling, psycho-social rehabilitation or case management fields.

Valid driver's license with acceptable driving record
