Requirements

The ideal candidate will possess the following:

• Bachelor's degree from an accredited school in counseling, social work, psychology, marriage and family counseling, or a closely related field.
• Training and experience in mental health and medical services are required.
• Must possess a valid driver's license.

We are seeking a Community Health Liaison to join our team in our Sayre, PAlocation.

Earn $18.28/hour.

Position Details:

The Merakey Community Health Liaison will assist in the development and ongoing continuity of collaborative relationships with all medical and psychiatric sites and programs in the community. The Liaison will provide community outreach, education, and other support services to the targeted populations to improve awareness about designated program(s) and resources. The Liaison will be an advocate who closes the gap between individuals receiving services, health care providers, health agencies, and other community-based resources by providing health care assistance, information, and education.

The Community Health Liaison will act as a link between the mental health and the health-care systems. Specifically, they will provide "cooperative" case management services to individuals with serious and persistent mental illness who are experiencing long term medical complications. Areas of focus will include increasing an individual's ability to function within their home community, reducing the need for more intensive services, developing and maintain independence, strengthening social supports, and over enhancement of the person's quality of life. Responsibilities will include assessment, service planning, linking, and monitoring psychiatric/medical status, support network building, problem resolution, obtaining resources, education, and advocacy.
