UPMC Your Care is hiring a Community Health Worker working Monday - Friday daylight shifts.
A Community Health Worker (CHW) is an integral part of the UPMC Your Care interdisciplinary team, dedicated to providing comprehensive and coordinated care to high-risk members of the UPMC Health Plan who have complex medical and social needs. The CHW collaborates with a range of health professionals, including physicians, social workers, pharmacists, and behavioral health specialists. The CHW's mission is to assist members and their families in reaching their health objectives, enhancing their quality of life, and minimizing unnecessary hospitalizations and emergency department visits.

Responsibilities:
- Perform home visits to evaluate members' health status, living environment, social support, and obstacles to receiving care. Offer health education and coaching on managing chronic diseases, adhering to medication regimens, practicing self-care, and utilizing preventive services.
- Help members access, schedule, and navigate health and social services, including primary and specialty care, mental health services, substance abuse programs, housing, food, medication, transportation, and legal aid.
- Support members during telemedicine appointments, addressing technical issues and serving as a telepresenter in the member's home.
- Aid members with specific non-medical challenges that impact their health and access to care. Ensure effective communication and coordination among the UPMC Your Care interdisciplinary team, care management staff, members, their families, and care providers throughout the continuum of care. Document and relay patient information.
Qualifications:
- High school diploma or equivalent. Certification as a community health worker or equivalent experience in community health, social services, or outreach. Knowledge of the local community, culture, and resources.
- Ability to establish rapport and trust with diverse populations, especially those with low income, limited literacy, and multiple chronic conditions. Excellent communication, interpersonal, and problem-solving skills. Proficient in basic computer skills, such as Microsoft Office, email, and internet. Flexible, adaptable, and willing to work in a fast-paced and dynamic environment. Solid organization skills and ability to set priorities and to schedule time efficiently. Self-directed and flexible to meet the needs of our members.
- Valid driver's license and reliable transportation. Proficiency in using electronic health record systems and other software applications. Has personal vehicle and ability to travel to patients' homes and other community settings as needed.
