Responsibilities
Coordinate access to health-related services by assisting clients with utilizing available resources.
- Identify potential clients, complete initial needs assessment, and maintain follow-up to ensure appropriate guidance to health services.
- Provide informal counseling, education, and social support to clients.
- Assist clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments,
and assisting with completion of medical program applications. - Develop and maintain strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).
- Maintain accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.
Education:
- High School diploma equivalency with 2 years of cumulative experience OR Associate'
degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
- Excellent written and verbal communication skills preferred.
- Driver's License and/or reliable transportation preferred.
- Bilingual Spanish or Arabic preferred.
- Familiarity with community resources preferred.
