Summary:
Serves as a liaison between the community and the health care, government, and social services systems to provide clients with the psychosocial support needed to cope with chronic, acute, or terminal illnesses and reduce infant and maternal mortality. Collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs. Advocates for clients or patients to resolve crises. Facilitates access to resources and services to improve service delivery quality and cultural competence.
Formal Education Required:
a. High School Diploma or equivalent
Experience & Training Required:
a. Experience working in health fields such as hospital system, pregnancy health, public health, community-based health or social service agencies, referral coordination, peer recovery support, or similar health professions preferred.
b. Completion of an approved Certified Community Health Worker Training Program preferred. (Must complete this training)
Other Skills, Competencies and Qualifications:
a. Ability to complete Certified Community Health Worker Training Program.
b. Basic computer skills
c. Must pass background check and drug test
d. Knowledge of basic medical terminology
e. Ability to work with diverse communities and different races, cultures and classes
f. Ability and willingness to provide emotional support, encouragement and motivation to clients
g. Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity
h. Ability to obtain and record vital signs, and recognize changes in health status
i. Second language skill is a plus
Level of Physical Demands:
a. Light: Exerts up to 20 pounds of force occasionally and/or up to ten pounds of force frequently, and /or a negligible amount of force continuously.
$15.30/hr - $18.36/hr
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers.
