Job Description

PURPOSE OF POSITION
The Growth and Community Engagement Specialist (Community Liaison Worker Level Ill} is responsible for patient growth and retention across the Gotham Health enterprise. He/She will develop and implement community outreach activities to drive patient referrals, educate the community about Gotham Health Services and showcase the brand.

SUMMARY OF DUTIES AND RESPONSIBILITIES
• Develops and participates in events and other community partnership activities to establish patient referral networks within and outside all Gotham Health sites.
• Recruits new patients for all Gotham Health Centers and sites by attending community events and local meetings.
• Spearheads special projects as needed and serves as the key liaison with community partners, Gotham sites, staff and site leadership
• Researches and initializes key business relationships to help drive and support department growth initiatives
• Coordinates and helps schedule newly established patient appointments across the continuum of care for Gotham Health.
• Responsible for reporting, updating and documenting individual referral metrics
• Responsible for overseeing individual budgets and reporting of all expenditures
• Links Gotham patients with community-based services through local organizations.

• Assists in data collection through community assessment surveys and partnerships.
• Creates partner agreements with community-based organizations, schools and neighboring agencies for the process of referring new patients to Gotham Health.
• Flexible nights and weekends to accommodate outreach and networking events
• Other duties as assigned

Minimum Qualifications

For Appointment to Level I
1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or
2. Completion of an approved six month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in "1" above.

For Appointment to Level II
In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires:
1. Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or
2. Education and/or experience equivalent to "1" above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above.

For Appointment to Level III
In addition to meeting the qualification requirements of Assignment Level II, appointment to Level III requires:
1. One additional year of full-time experience as described in "1" above.

For Appointment to Level IV
In addition to meeting the qualification requirements for Assignment Level III, appointment to Level IV requires:
1. One additional year of full-time experience.

Department Preferences

QUALIFICATIONS FOR THE JOB: CERTIFICATION(S)/LICENSE(S):
EDUCATIONAL LEVEL:
Associates Degree or higher preferred

KNOWLEDGEABLE IN:
Microsoft Word
Microsoft Excel
Microsoft Power Point
Community Relations experience preferred
Comparable experience in health plan community relations, provider relations or pharmaceutical sales. Understanding of provider marketing, and knowledge of NYC Cares and/or the NY State of Health a plus.

Must be comfortable making presentations and public speaking addressing providers, small business owners, community influencers, etc.

Bilingual preferred in one or more of the following:
• Spanish

• French Creole
• Arabic
• African Dialects
• Russian
• Polish/Albanian
