Overview

Summary:

The Community Paramedic 2 may provide patient care in 911/EMS operations, inpatient, ambulatory non acute, ambulatory acute and post-acute care settings. Provides direct patient care to sick and injured patients in all age groups requiring basic and or advanced life support skills. Expands access to health care through in-home patient visits and/or community-based services. Works in cooperation with other stakeholders/key medical providers. Serves as a trained "navigator" of community resources. Provides patient and outreach education. To work EMS operations, candidates must have at least one year of experience with Novant Health New Hanover EMS and be fully released to staff a quick response vehicle.

Responsibilities:

Essential Functions 1. Demonstrates refined assessment skills to identify patients' acute needs, past medical history, and social determinants of health. Collaborates with the patient, patients chosen support system, and interdisciplinary health care team to develop an individualized plan of care. Constantly reassess to determine if modifications to patients plan of care are required based on patients' condition, capabilities, and needs. Provides routine clinical care such as starting IV's, providing injections, administering, and monitoring medication therapies, performing wound care, diagnostic cardiac monitoring, phlebotomy, etc. Ensures completion of medical orders, interventions, assessments, and electronic medical record documentation. Provides telehealth services including telemedicine, virtual visits, and remote patient monitoring. 2. Provides proactive care that is intended to help improve the overall health and wellness of program participants. Requires excellent rapport, understanding, and communication with patients, patients chosen support system and patients health care team. 3. Expands access to non-urgent health care through in-home patient visits and/or community-based services. Common services will include assessment (vital signs, blood pressure, labs such as glucose levels, diagnostic cardiac monitoring, medication compliance and reconciliation, adherence to discharge instructions, etc.), treatment (wound care, medication administration, post operative follow-up, etc.), prevention (immunizations, home safety check, health teaching, etc.), and referral (mental health, medical and social services, etc.). Thereby producing better health outcomes and reducing the number of ambulance transports, visits to the emergency department, and hospital readmissions. 4. Works in cooperation with other stakeholders/key medical providers to offer coordinated and integrated care (physician offices, home health agencies, hospice, long term care facilities, public health department, social services, etc.). 5. Serves as a trained "navigator" of community resources, connecting patients to agencies and services they may not be aware of and reduces barriers to care. 6. Consistently demonstrates professional behavior and performs proactive interventions which support the Mission, Vision, and Values of Novant Health NHRMC and this department. Responds to patient concerns. 7. Functions as physician and hospital extenders by utilizing advanced protocols/procedures approved by the Medical Director and North Carolina Office of Emergency Medical Services. 8. Provides outreach education to the public (i.e., Hands-Only CPR, car seat and bike helmet clinics, etc.). 9. Participates in peer case reviews. 10. Maintains Paramedic scope of practice and all knowledge, skills, and competencies required by North Carolina Office of Emergency Medical Services, department Medical Director, and department policies / procedures. 11. Acts in the capacity of a 911 Staff Paramedic as needed. Maintains all knowledge, skills, and competencies to perform this role (Refer to Staff Paramedic Job Descriptions, NHREMS-Field Operations). 12. May perform moderate or waived point of care testing per CLIA guidelines. 13. Demonstrates standards of performance that supports our Cause to create a healthier future and bring remarkable experiences to life. Performs other duties as assigned.

Qualifications

Credentials:
Essential:
* PreHospital Trauma LifeSupport
* Drivers License Class C
* Paramedic

Education:
Essential:
* Technical/Vocational Diploma

Other information:

Education: High School Diploma or GED Equivalent required. College degree preferred. Completion of an approved accredited Community Paramedic program preferred.

Experience: Minimum of 5 Years Paramedic experience required. 1 Year of service at NHNHRMC EMS required.

Refer to the Life Support Training Policy NH-HR-3096.

Licensure/Certification: Current North Carolina Paramedic Certification required. Basic Cardiac Life Support required. Advanced Cardiac Life Support required. Prehospital Trauma Life Support or International Trauma Life Support required. Community Paramedic Certification preferred. Completion of an accredited and approved Community Paramedic Training Program within one year of hire date required.

Additional Skills/Requirements (required) Current North Carolina Driver's License and must meet requirements of the Medical Center's Insurance Carrier. Must meet general hospital driving requirements. Emergency Vehicle Operations (EVO) required to drive during emergency situations.
