Description:

Assist participants and their families in managing bereavement stemming from a life-threatening disease or chronic medical, physical, or developmental conditions of the participant. Children and adolescents who have a terminal illness or persistent medical/developmental concerns, together with their families, deal with grief and loss in different ways. They may require different forms of care throughout time, such as therapy, support groups, and other services. Participants receiving services from a hospice care provider may also be eligible for HCBS Counseling and Support Services. Your responsibilities may include preparing treatment plans, discharge plans, follow up care programs, and providing therapeutic crisis intervention or emergency services as required.

Responsibilities:
  • Through a collection of interactions with patients and other health care providers, collect and analyze data regarding the patient's psychosocial history and current environment, and identify anticipated discharge needs.
  • Ensure that treatment plan goals and interventions effectively address acuity, patient management issues, and target behaviors, ensuring the goals and objectives are measurable.
  • Ensure individual, family, and group therapy documentation is completed accurately and timely.
  • Provide individual therapy patients, ensure therapy interventions are directly related to treatment plan goals and objectives
  • Meet with patients weekly to assess response to treatment, level of functioning and treatment needs
  • Assist patients in setting appropriate goals and discuss ongoing discharge plan with clients
  • Communicate patient's progress and readiness for discharge to members of treatment team
  • Regularly update service plans, obtain medical consenter signatures, and ensure plans reflect members' evolving needs.
  • Adhere to all documentation requirements, submission of progress notes, service plans, and service provider logs in a timely manner.
  • Submit Progress Notes within 3 days of service provision, so that approval from supervisor is provided by the fifth business day following service provision
  • Other duties as assigned.

Minimum Qualifications (as designated by New York State):
  • Licensed Clinical Social Worker (LCSW) or
  • Licensed Master Social Worker (LMSW) or
  • Licensed Mental Health Counselor (LMHC) or
  • Licensed Creative Arts Therapist (LCAT) or
  • Licensed Psychologist

Experience Required:
  • Minimum three years of experience dealing with medically fragile populations AND
  • Minimum one year of clinical experience working with pediatric patients
    • Preferred: involving end-of-life care
