GENERAL SUMMARY:

This position is responsible for the managing the CREATION Health Transportation Program, which involves scheduling, transporting patients, networking with local provider offices, teaching the principles of CREATION Health to patients while on-board and database management.

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES :

• Effectively plan routing and schedule appointments (pickups/drops)

• Create and maintain a database to facilitate reporting on the Transportation project

• for Corporate and Accounting

• Educate patients regarding CH principles

• Demonstrates ability to function in an 'as needed' capacity outside the scope of the CH program within the Marketing/Communications/Business Development department

Qualifications
EDUCATION AND EXPERIENCE REQUIRED:

• High school diploma

• 3 years of driving/courier experience

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

• CPR/First Aid
