For each responsibilities listed, indicate the performance level achieved. 1 Performs exams following modality specific protocols resulting in high quality images using appropriate imaging techniques. 2 Demonstrates working knowledge of any and all modality specific ancillary equipment such as laser camera, fluoroscopic equipment, contrast injectors, QC phantoms, Gus cleaning units, CR readers PAC's/RIS workstations, CAD workstations, dose calibrators, and flood sources. 3 Appropriate storage, handling and administration of modality specific contrast agents including barium, iodinated oral contrast, and intravenous contrast agents. 4 Maintains documentation and insures accuracy of all pertinent patient and procedural data such as patient history, exam title, patient demographics, laterality markers, and modality specific questionnaires and screening tools. 5 Prepares patient for procedure: providing instructions to obtain desired results. Gain cooperation and minimize anxiety as per patient education standards. 6 Applies safety principles to minimize risks to patient, self, and others. Following establish standards such as Alara, Image Gently, and MRI safety. 7 Conducts ongoing patient assessment including monitoring of the patient's condition and assuming the responsibility for provision of physical or psychological needs during procedures ensuring patient safety. Provides pre and post procedure patient education. 8 Assists as needed to perform modality specific QA/QC procedures insuring equipment operates within manufacturers and regulatory guidelines. 9 Assumes responsibility of assigned area assuring all rooms are clean, stocked with supplies, and meets the standards set by each modality. 10 Participates in modality specific PI Plan. 11 Contributes to the efficient operation of the department through communication either written or verbal with lead technologist training to equipment, patients and other staff members. 12 Demonstrates computer literacy and application with computer programs that include HIS, RIS, PACS and ancillary CAD applications. 13 Displays analytical and basic problem solving skills. 14 Follows all time and attendance policies. 15 Attends educational programs related to job responsibilities as required. 16 Maintains all licenses and professional accreditation including society specific CME's as required. III.
