Community & School Health Initiative

Responsibilities

Under the direction of the Director of the Center for Healthier Communities and with guidance from the Program Manager and support from the Program Coordinator, the Department Support Assistant is responsible for outreach & education and monitoring & improvement for an assigned grant-funded program and corresponding activities in order to improve community health, in a respectful and dignified manner.
  • Build and maintain partnerships with local schools, organizations and other relevant entities to foster collaboration and extend outreach for the assigned program.
  • Interact with diverse members of the community to help achieve program goals and objectives.
  • Conduct activities to promote in-person or virtual outreach, public awareness and/or recruitment at community sites (ex. schools, faith-based organizations and other community-based organizations).
  • Identify and secure sites for presentations (in-person or virtually), outreach events and/or distribution of educational materials.
  • Assist the Program Coordinator in developing an outreach calendar, as needed.
  • Create program promotional or educational materials as appropriate for the program (ex. newsletters, brochures, flyers, handouts).
  • Enter and maintain accurate outreach data & records on all outreach and education activities conducted.
  • Support the Coordinator with data for completion of internal or external progress reports in a timely manner (ex. preparing data).
  • Assist the Program Coordinator with tracking incentive inventory, if necessary.
  • Effectively communicate with Program Coordinator and relevant staff to troubleshoot any program challenges in a timely manner.
  • Engage in continuous learning and gather input from community members and partners to improve program effectiveness.
  • Communicate regularly with program staff and program stakeholders to keep all members up-to-date with relevant program information.
  • Attend all scheduled meetings with the project team, as needed.
  • Supports training of new employees/interns/volunteers to ensure proper operation of the program, as needed.
  • Performs other duties as assigned.


Qualifications
  • One year experience in healthcare or community based organization.
  • Experience working with community outreach or public health preferred.
  • Bilingual in Spanish/English preferred.
  • High School graduate or GED equivalent required.
  • BSW or BA in social work or related field preferred.
  • Working knowledge of Microsoft 365 and Google Workspace products, Zoom and Canva Presentation and public speaking skills.
  • Knowledge of local community agencies and resources.
  • Ability to organize resources and establish priorities.
