Job Description

SUMMARY OF DUTIES AND RESPONSIBILITIES:

The Care Coordination Data Manager is a grant-funded position and will work within the NYC Department of Social Services/Department of Homeless Services (DHS) Health Office, under the direction of the Director of Health Planning and Evaluation. The Data Manager will be stationed at DHS's central office in downtown Manhattan. They will act as a project liaison between H+H and DHS and will be the lead data manager and coordinator for a complex care coordination program serving the most vulnerable New Yorkers experiencing homelessness with multiple health conditions. They will be responsible for developing, implementing, monitoring, and analyzing data and electronic communication systems, workflows, and client-level characteristics, interventions, and outcomes.

The Care Coordination Data Manager will be responsible for the following:
1. Coordinate with the DHS and H+H project lead(s) to implement a Complex Care Coordination Program, including hiring staff, coordinating agreements, and fulfilling all reporting requirements.
2. Develop and maintain data systems to track referrals, interventions, outcomes, and other relevant demographic and social services data for the Complex Care Coordination Program, including regular quality assurance activities.
3. Coordinate and support the onboarding of Complex Care Coordination Program staff, including coordination of necessary training for Care Management staff on data collection and documentation systems.
4. Develop program referral and care coordination workflows, data monitoring, and program implementation reports. This includes identification and escalation of project barriers, proposing solutions, quality improvement projects, and modifying the project plan or other program materials.
5. Identify and implement data systems to assist with referrals from inpatient hospital settings in collaboration with the DHS institutional referral program.
6. Perform other duties as assigned.

Minimum Qualifications
1. A Masters degree or advanced clinical degree from an accredited college or university and one (1) year of experience gathering and documenting business, clinical, and/or functional requirements using interviews, document analysis, workshops, surveys, site visits, business process descriptions, scenarios, business analysis, task and workflow analysis or related in a clinical, allied health, non-allied health, behavioral health, information technology, information services, business, analytics or related area; or, one (1) year of experience in a relevant clinical and/or healthcare administration role/function; or
2. A Baccalaureate degree from an accredited college or university and three (3) years of experience as described above; or
3. A satisfactory combination of education, training, and experience.
4. Specialty certification(s) issued by a national commercially available program, state, professional society, academic or technical institution in an area(s) listed above, may be credited on a month-to-month basis toward the required work experience for a total of one (1) year.

Department Preferences
• 4 years' experience of progressively responsible experience in project and program management, execution, and implementation, two (2) years of which must have been in the administration, management and/or execution of projects and programs of various complexities
• Familiarity with social services, health, and/or homeless services systems
• Familiarity with NYC DHS shelter operations, specifically single-adult shelters and street homeless programs
• Ability to assume a leadership role in managing projects and associated risks related to projects, systems, and business analysis
• Effectively interacts with senior management and interdisciplinary stakeholders.
• Strong problem-solving, analytical, and critical thinking skills
• Excellent verbal and written communication skills
• Strong ability to anticipate needs and proactively address issues
• Excellent organization and attention to detail
• Must work with the following computer programs/software:Microsoft Suite, including PowerPoint, Excel, Outlook, Visio, Word, Adobe Suite, SAS, R, or other programming languages
