Job Description

Assignment Level 1:
Reporting directly to Associate Director of Chronic Disease Management, the Dietitian Level 2, is responsible for providing direct comprehensive Nutritional Education and Counseling to Pediatric and Adolescent patients

AREAS OF RESPONSIBILITIES
1. Provide nutrition education, assessment, documentation and intervention to patients.
2. Develop care plans for high risk individuals.
3. Provide nutritional educational handouts based on identified areas of concern.
4. Document patient encounters in EMR.
5. Participant in team meetings on chronic diseases.
6. Participate as a Health Coach for Diabetes Prevention Program.

SUMMARY OF DUTIES AND RESPONSIBILITIES
1. Ability to provide direct patient care to pediatric and adolescent patients.
2. Conduct a complete in-depth comprehensive nutrition assessment and evaluation, reviewing laboratory values, anthropometric measurements, growth charts and other appropriate objective nutrition requirements for pediatric, adolescent and adult patients and document in the medical record.
3. Include parents in counseling of patient's nutritional needs, nutrient requirements, care plan, meal planning, BMI evaluation and risk for nutritional deficiencies; ensuring proper follow-up
Appointments.
4. Consults with physicians and other healthcare professionals concerning patient's care with recommendations for medical nutritional therapies.
5. Knowledge of pediatric and adolescent Medical Nutrition Therapy.
6. Develops, Implements, Coordinates and Evaluates a Weight Management Program for overweight and obese pediatric and adolescent patients and ensures proper follow-ups.
7. Conduct educational group classes on weight management.
8. Develops appropriate nutritional educational handouts for pediatric and adolescent patients.
9. Participates in outreaching to patients that have missed appointments.
10. Attend weekly Chronic Disease Meeting and other monthly meeting assigned by Supervisor.
11. Perform monthly chart review and identify outliers of assigned patients.
12. Maintains current nutrition knowledge through participation in seminars/professional continuing educational meeting.
13. Perform all tasks with professionalism, knowledge and caring helpful attitude.
14. Ability to rotate between different sites and work flexible hours.
15. Perform other duties as stated by supervising team.

Minimum Qualifications

Qualification Requirements:

For appointment to Assignment Level I
1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or,
2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Level II
1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Levels III and IV
1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility:
Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.

QUALIFICATIONS FOR THE JOB:
CERTIFICATION(S)/LICENSE(S):

1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or registration or eligibility for registration by the Commission on Dietetic Registration
(CDR) as a Registered Dietitian or New York State Certification as a Dietitian- Nutritionist.
2. 1-2 years' experience or more working in an Ambulatory Care Setting with pediatric and adolescent patients.

EDUCATIONAL LEVEL:

Bachelor's degree.

KNOWLEDGEABLE IN:

Medical Nutrition therapy, nutrition counseling, documentation, and assessments.

EQUIPMENT/MACHINES OPERATED:

PC.

COMPUTER PROGRAMS/SOFTWARE OPERATED:

Microsoft Office.

YEARS OF EXPERIENCE:

1-2 years.
