Summary:
Leads, develops, implements, and provides oversight for all aspects of care management and transitions throughout the continuum of care to drive better care and outcomes for patients. Provides oversight of care management initiatives, projects, and programs developed to meet the needs of patients and the communities we serve and maintain compliance and performance with required regulations. Provides oversight for operational performance of care management: profit and loss/financial performance, involved in developing and managing the budgets; ensures strong program development and performance with effective collaboration amongst the care team.

Evaluates the effectiveness of care management programs and collaboration with the clinical team to develop processes, initiatives, and programs to address gaps in care.

Works collaboratively with the VP, Integrated Care Management and Population Health to support Summa Health's population health strategy to meet organizational goals related to quality outcomes, patient experience and cost-efficiency, supporting system-wide strategic initiatives focused on complex care management.

Collaborates with the VP, Integrated Care Management and Population Health and engage senior and physician leadership, administration, professional, clinical, and other staff throughout the organization to implement population health programs and promote a culture consistent with the health system's mission.

1. Formal Education Required:
a. Bachelor's degree in nursing; or a degree in public health, clinical science, business, and relevant health care experience.
b. An advanced degree (ex: MSN, MBA, MPH,) in a relevant field or equivalent experience.

2. Experience and Training Required:
a. Five (5) years of progressively responsible experience in healthcare management or in a in a variety of clinical settings and roles that has provided leadership skills, to include:
i. Population health management or managed care experience
ii. Demonstrated success at building effective cross-functional and multi-disciplinary teams
iii. Process improvement experience or training

3. Other Skills, Competencies and Qualifications:
a. Demonstrated ability to constructively challenge established ways of doing things to lead improvement and change
b. Strong informatics and analytic background
c. Demonstrates in-depth knowledge of healthcare management and managed care, and a solid understanding of the continuum of care.
d. Possesses knowledge in the following: quality improvement; population health; care management; information systems and their applicability to clinical effectiveness/outcomes management; contemporary management principles; accreditation standards; value-based reimbursement models.
e. Understands data collection and analysis.
f. Possesses excellent writing, organization, and facilitation skills.
g. Demonstrates management and leadership experience.
h. Demonstrates superb relationship building skills.
i. Presents a professional and personal demeanor characterized by the ability to serve as a facilitator and consensus builder.
j. Proficient in Microsoft office software programs and experienced in constructing and using project plans.
k. Ability to adapt new models of care, when appropriate, to variations in population subsets for targeted conditions
l. Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity

a. Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently.
$49.29/hr - $73.94/hr
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers. Please keep in mind that the range that is listed is the full base salary range. Hiring at the maximum of the range would not be typical.
