Job Overview
We are seeking a dedicated and experienced Director of Nursing to join our team. The ideal candidate will oversee the nursing department, ensuring the delivery of high-quality care to our residents. This role requires strong leadership skills and a deep understanding of nursing practices.

Responsibilities
  • Performs thorough health assessment on all new residents upon admission to the community.
  • Monitor status of resident's health conditions and initiate actions in response to changing needs and read nursing communication logs, nursing 24 hour report and other documentation daily.
  • Monitor the eMAR to ensure quality assurance (Appropriate medication documentation and administration system for each resident. Monitor all medications and prescriptions. Update medication system as needed.)
  • Utilize Electronic Health Record system and Pharmacy Portal to review new orders, link medications received to current orders and follow up on medications that are sent from pharmacy without valid orders in the eMAR.
  • Conduct quarterly re-assessments of resident's health needs, including fall assessments, braden skin assessments, elopement/wandering assessments and ensure that quarterly chart audits are performed.
  • Reacts immediately to suspected resident abuse including a thorough investigation and notification of appropriate parties.
  • Performs all necessary verbal and written disciplinary actions required of supervised staff, and ensures documentation and process meets Company policies and procedures.
  • Supervise and lead nursing staff, including hiring, training, and performance evaluations.
  • Serve as a liaison between community, applicants' family and appropriate medical personnel. (Assist community to problem solve when a change in the resident's health status occurs. Coordinate with other community health care professionals, such as the home health agency, to acquire use of their expertise when necessary.)
  • Assure TB tests are completed on time for all new residents.
  • Assure that annual TB surveys are completed for residents and documented in their charts.
  • Attend Director Meetings as required.
  • Develop appropriate service plans and evaluate changes in care needs.
  • Determine if resident is appropriate for admission and remains within our level of care.
  • Attend to all resident needs if they arise.
  • Monitor care of resident to include, but not limited to ADL's medication side effects, activities, meals, complaint problems, special requests, suspected abuse, injuries etc.
  • Follow up weekly with residents who are out of the facility (hospital, skilled nursing, rehab) and ensure that Sales Leader, BOM and Executive Director are aware of current status.
  • On call duties on a rotating basis - responsible to triage emergency care needs, assure staffing needs are met, and respond to other miscellaneous problems that may arise.

Qualifications
  • Valid/Active LPN/RN license
  • Supervision and leadership experience required.
  • 5+ years of experience in Assisted Living performance preferred
  • Strong understanding of laws and regulations related to assisted living communities.
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team.
  • Strong understanding of Medicaid regulations.
  • Knowledge of care planning processes and documentation review.
  • Ability to prioritize tasks effectively and handle multiple responsibilities simultaneously.
