Job Description

PURPOSE OF POSITION

The Director of Rehabilitation Services- Service Delivery will be responsible for providing oversight to a team of Assistant Director of Rehabilitation as well as rehabilitation staff who provide oversight to a clinical team in the coordination and rendering of care delivery in the Certified Home Health Agency (CHHA). The Director of Rehabilitation will be responsible for the delivery of competent, effective and safe rehabilitation services, including but not limited to physical therapy, occupational therapy and speech language pathology. In addition, the Director of Rehabilitation Services- Service Delivery will engage in efficient advisement, coaching and mentoring strategies to the members of the care team in order to meet clinical and professional goals. The Director of Rehabilitation- Service Delivery will stay abreast of industry trends and regulatory guidelines that govern the home health care team and clinical practice.

Duties & Responsibilities

ESSENTIAL DUTIES AND AREAS OF RESPONSIBILITIES
1. Responsible for designing, implementing and evaluating the referral processes of rehabilitation patients to home care.
2. Teach and supervise staff to gain knowledge and skills to appropriately carry out managerial and operational tasks.
3. Develop and review periodically the criteria for referring patients into service and monitor the appropriateness of referrals in conjunction with the Transition Care Nurse (TCN) and Associate Executive Director (AED).
4. Participates in orientation for Assistant Directors of Rehab and other rehab staff were indicated and in conjunction with the Education department/Clinical Education team.
5. Evaluates and ensures competency requirements including semi-annual to annual field evaluations are met for all clinical staff.
6. Responsible for directing and supervising the rehabilitation staff and acting as administrative and clinical resource to them.
7. Monitor the performance of direct reports and conducts performance appraisal as per Human Resources policy and procedure.
8. Ensures the progressive disciplinary process is initiated for all staff and ensure disciplinary action up to and including termination where indicated.
9. Interpret program services and policies to general public and other health services, providers and recipients of Home Care at NYC Health and Hospitals CHHA and ensure staff apply these policies appropriately.
10. Monitors rehabilitation staff (Occupational Therapist, Physical Therapist, Speech Language Pathologist) assignments, caseloads, service utilization, and team productivity.
11. Provides direct supervision to field staff, including on-site supervisory visits in the absence of direct reports.
12. Maintain and report statistics of departmental operations to support strategic planning, program evaluation and budgeting, organizes, analyzes and reports these data on a regular basis.
13. Conducts periodic documentation management and review, addresses issues and deficiencies with respective staff, and devises a plan of corrective action or proactively resolve the findings.
14. Ensures direct reports monitors all documentation is timely and accurate, including OASIS, plans of care and daily visit documentation.
15. Performs field visits for the purpose of evaluating rehabilitation staff and participates in family conferences and may be asked to perform nursing visits in times of increased demands or scheduling issues as needed.
16. Identifies appropriate staffing patterns and needs of assigned staff based on productivity and team census and provides updates on census, staffing, team productivity and other reports as requested.
17. Coordinates appropriate communication and documentation between patients, agency, medical professionals, and referral sources.
18. Assists with assigned performance improvement data collection.
19. Implements performance improvement plan (PIP) and Quality Assurance Performance Improvement (QAPI) plan as needed.
20. Participates in bimonthly to monthly QI meetings/Quality Committee meetings and clinical record review when needed.
21. Provides direct field supervision to staff as well as in office staff in charge of care management and/or coordination.
22. Ensures compliance with state and federal home health regulations as well as NYCH+H Home Care Policies and Procedures.
23. Demonstrates sound judgment by taking appropriate actions regarding suspected violation of corporate compliance regulations.
24. Reports all suspected compliance issues to AED, Compliance Officer or Compliance Hotline.
25. Ensures direct reports participate in case conferences, utilization review and utilization management of admitted cases and Interdisciplinary Team conferences as needed.
26. Assigns staff to Interdisciplinary Team as needed to review care delivery and promote interdisciplinary management of patients admitted to the home care department as needed.
27. Participates in mock survey, accreditation visits and other external organization reviews.
28. Monitors key metrics on the clinical and operational dashboard in EMR and performs data collection activities and related data analysis of the home care program.
29. Continuously analyzes work processes and makes suggestions for improvement.
30. Stays abreast of home health and management issues and trends and shares information with staff.
31. Works with other members of the Interdisciplinary Team and other Directors/Assistant Directors to develop appropriate interventions in order to achieve clinical and functional goals.
32. Liaises with schedulers, TCNs to coordinate patient referrals, patient care and patient assignments.
33. Assures the formulation, implementation and updating of an individualized care plan for each patient admitted to the CHHA.
34. Develops and manages special programs (e.g. Joint replacement and fall prevention) and other strategic initiatives for business development and increasing the rehabilitation caseload.
35. Conducts case load reviews and chart audits on a weekly to bi-weekly basis for all clinicians assigned to the team and develops interventions to correct identified deficiencies.
36. Performs other duties as assigned.

Minimum Qualifications

QUALIFICATIONS REQUIREMENTS
1. Current New York State License and Registration as a Licensed Physical Therapist; and
2. A Baccalaureate Degree from an accredited college or university in physical therapy approved by the commission on accreditation in Physical Therapy Education (CAPTE) of the American Physical Therapy Association (APTA); and
3. Five (5) years of professional experience in administering physical therapy at least two (2) years of which must have been in an administrative, managerial or supervisory capacity.
