Full Time, Monday - Friday

The Ridge Behavioral Health System is a 110-bed hospital located in the heart of the Bluegrass, Lexington, KY. The Ridge provides psychiatric and substance use disorder treatment for children, adolescents, and adults. The Ridge offers Partial Hospitalization, Intensive Outpatient Programs, Individual Counseling, as well as Medication Management for all ages. We have provided behavioral health services to over 92 Kentucky counties for more than 39 years.

The discharge planner works directly with the patients in aftercare planning, setting up appointments, and facilitating placement referrals for discharge as appropriate. The discharge planner actively participates with treatment team/external case managers/managed care organizations, refers professionals, implements, and supervises the discharge planning. Follows and abides by company policies, procedures, and Corporate Compliance Program, as applicable to role and responsibilities.

Job Duties Include:
  • Assures discharge planning is reviewed at the time of admission and updated to facilitate effective discharge planning.
  • Ensures safety plan is in place and all weapons including guns, knives, OTC, medicines, etc. are locked up and or secured.
  • Works closely with the social workers to integrate psychosocial management of patient/family needs.

Qualifications

Education
  • Bachelor's degree in Social Work, Psychology, or other Mental Health field preferred.
  • Minimum associate degree with 5 years mental health experience required.
Experience
  • One to two years of experience in a psychiatric hospital is preferred.
  • One to two years of experience in Utilization Management preferred.
Certifications/Licensure
  • Crisis intervention training required (training offered upon hire)
  • CPR certification (training offered upon hire).
Other Skills
  • Excellent written and verbal communication skills.
