Shift : Monday-Friday 8am-4:30pm
The community you'll be caring for: AdventHealth Waterman
The role you'll contribute:
Responsible for continuity of care across the continuum with emphasis on quality patient outcomes and cost-effectiveness through decreased length of stay, and control of resource utilization. Provides Patient/family instruction, scheduled physician appointments, any additional discharge collaboration/coordination in the hospital's EMR and coordinates activities according to patient's need.

• Good Report with physicians/providers
• Follows established criteria for utilization of services to ensure denials are avoided
• Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self-care, nursing facility, home health, physical / psych facilities
• Acts as liaison with physician to assure appropriate and complete documentation
• Prioritizes activities and completes work assignments on a timely basis while functioning with minimal supervision
• Assists physicians by providing information, suggestions, alternatives, and timely referrals for discharge planning
• Provides continuity of care by coordinating the flow of information between hospital departments
and outside organizations
• Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self-care, nursing facility, home health, physical / psych facilities
• Reviews patient medical records for PI / Risk Management per established criteria and provides appropriate documentation of adverse patient occurrences
• Demonstrates ability to request for care from referring sources within the community in a timely manner.
• Provides information to physicians and the community on the benefits and alternatives home health and support service programs can provide
• Assists physicians, patients, and families with appeal process
• Participate effectively in Performance Improvement activities as requested or needed
Qualifications
The expertise and experiences you'll need to succeed :
Minimum qualifications :
• LPN
• Three years LPN health care experience
• Strong communication skills with the ability to effectively collaborate with other members of health care
• Appropriate documentation skills
Preferred qualifications:
• 5 years' clinical experience
• Hospital Case management discharge planning experience
• Home care or skilled nursing facility experience
• Computer Skills
