Primary Responsibilities:
The schedule for this position is Monday - Friday, 8am - 5pm. This is a field based position visiting our members in their homes, community based housing, and long-term, skilled and acute care facilities in the Washington DC area. You'll need to be flexible, adaptable and, above all, patient in all types of situations.
This is high volume, customer service environment. You'll need to be efficient, productive and thorough dealing with our members over the phone. Solid computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.
You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.

Primary Responsibilities:
- Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
- Identify, initiate, and follow up on referrals for community based programs and providers; including financial, psychosocial, community and state supportive services
- Manage the care plan throughout the continuum of care as a single point of contact
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications:
- Current, unrestricted RN license in Washington DC
- 3+ years of experience in a hospital, acute care or direct care setting
- Demonstrated ability to type and have the ability to navigate a Windows based environment
- Access to reliable transportation
- Ability to work in a field based capacity in Washington, DC
Preferred Qualifications:
- BSN
- Certified Case Manager (CCM)
- A background in managed care
- Case management experience
- Experience or exposure to discharge planning
- Field based experience going into homes
