Overview
General Internal Medicine APP - Expert Diabetes Clinician

General Internal Medicine at DHMC Lebanon is seeking a qualified, enthusiastic APP with expertise in the care of patients with diabetes to join our team.

Key responsibilities/expectations:
  • Manage a small continuity panel of adult general internal medicine patients.
  • Provide expert Diabetes consultation for GIM patients who meet certain criteria-this would include new diabetes diagnoses with an A1c >9 or FBS >300, patients with high medical complexity and DM, Complicated insulin regimens, or high insulin requirements, Multiple DM medications, a history of gastric bypass, new CGM starts, etc.
  • Lead an annual QI project to improve the care of patients with diabetes in the section of General Internal Medicine.
  • Possess or commit to pursuing the credential as either board-certified in Advanced Diabetes Management (BC-ADM) or a Certified Diabetes Care and Education Specialist (CDCES) within three years of the hire date.
  • Maintain professional relationships with colleagues in endocrinology through clinical collaboration, scholarly work, and quality improvement projects.
  • Collaborate with an interdisciplinary team of fellow clinicians, pharmacists, nurses, nutritionists, and social workers to realize a holistic person-centered approach to diabetes management.

    • Graduate of an accredited APRN or PA academic program
    • National specialty certification as FNP, WHNP, PNP, ANP or certification through the National Commission on Certification of Physician Assistants (NCCPA)
