General Neurologist

CME Funds: $4,500; for license, DEA, Medical Staff dues, professional memberships
tuition and travel for conferences, etc. Prorated for FTE of 0.89 + $4,500.
Commencement Bonus: $25,000
Projected total compensation: $352,391 (base and 100% of citizenship bonus) plus productivity.
Schedule
4 or 4.5 days per week. Standard day or half-day) off to be mutually determined at time of negotiation of physician employment agreement. Currently, Wednesday or Thursday or perhaps Monday would be an option.
8 am to noon and 1pm to 5 pm (8 hours of clinical time/day)
Appointment durations would begin at 40-60 minute to allow for orientation and EMR. At an appropriate time (usually 2-4 weeks), appointment durations would be reduced to match standards for neurologists, i.e., 20 minutes for established patients and 60 minutes for new patients.
Typical productivity (based on the usual mix of new and established patients) will be 12 to 16 patients per day. In general, 2 to 4 new patients per day. Time for interpretation of diagnostic tests (e.g., EEG) is provided. However, administrative time is generally not provided at the specialty practice. Locations of care/schedule (specific days of week to be determined at time of
negotiation of physician employment agreement or commencement of employment, based on available space and operational needs at each site).
Preferred candidates have prior experience (at least 2-5 years) in broad spectrum of general neurology. Selected new graduates from residency or fellowship training may be eligible. Prior experience in a small hospital or CAH desired.
Must be comfortable with rural nature of practice/hospital, only one neurology colleague, solo coverage, and complement of specialists and diagnostic testing capacity. Special clinical interests/expertise to complement those of current neurologists and expand scope of services to our region. For example, clinical interests/expertise in one or more of the following: headaches, movement disorders, multiple sclerosis, extrapyramidal syndrome, stroke, epilepsy, dementia, and EEG's. Ability to interpret EEG studies desirable but not required.
Ability to perform Botox injections desirable but not required.
Team oriented, collaborative, and patient focused. High level of customer service (patient
satisfaction) is expected.
H1-B and J-1 visa candidates will be considered if excellent communication skills.
Job Highlights
Seeking a full-time (4.0 or 4.5 days/week) neurologist to join a highly productive, skilled, and popular neurologist and selected outreach clinic sites to help meet clinical demands for services and build a neurology service (system of 3 Critical Access Hospitals).
Provides office-based general neurology services to new and established patients (adolescents age 16 and older as well as adults), including medication management, education and counseling of patients and families, ordering diagnostic tests, referral to related other specialties (e.g., cardiology, orthopedics, pain management, pulmonology, sleep medicine) locally, referral to subspecialty neurologists at tertiary care centers, and communication with PCP's.

Partners with other members of neurology team (e.g., neurologist colleague, radiologist, EEG technologists, MA's and RN's, physical therapists). Infrequently provides consultation for inpatients (0-3 times/week). More often providers curbside often (unofficial) consultations to hospitalist and arranges to see patient in timely manner (

• Contributes 4% of Physician's earned compensation to 457(b) plan with an
annual maximum of $22,500 per IRS policy. Earned compensation includes base salary, Good Citizenship Bonus, and productivity incentive; excludes commencement bonus, relocation reimbursement, educational loan reimbursement, and housing stipend.
• Physician may make pre-tax contributions to 457(b) to supplement employer
contribution up to annual maximum.
• Illustration: If Physician's annual earned compensation was $250,000, we would
contribute $10,000 to 457(b).
• We also offers a 403(b) plan for employed physicians. However, such pre-tax
contributions are entirely self-funded by the physician up to annual maximum of
$20,500 per IRS policy.

Good Citizenship Bonus
• For each Fiscal Year of this Agreement, or a portion thereof, the Physician shall
also be eligible for a Good Citizenship Bonus in addition to Annual Base Salary
based upon achievement selected criteria. Typically, 3 or 4 criteria per
year, which may change from time to time, generally no more frequently than once annually in advance of a Fiscal Year.
• The maximum annual Good Citizenship Bonus per Fiscal Year is $15,000.
• The actual Good Citizenship Bonus earned by the Physician shall be determined solely at the discretion of the Specialty Practice Director and the Senior Vice President/Chief Medical Officer.
• Such bonus amount, if earned, shall be paid within 60 days following the
completion of the Fiscal Year.
wRVU Productivity Bonus
• In addition to Annual Base Salary, the Physician shall be eligible to receive additional compensation for high levels of clinical productivity as measured by wRVU for professional services personally performed by the Physician, such as evaluation and management (E&M) services for office visits and consultations, formal interpretation of diagnostic tests, and the performance of procedures.

Mileage Reimbursement
Consistent with policy, shall reimburse the Physician for mileage related to
outreach clinic travel (to/from sites).
The current rate is 62.5 cents/mile.
Diagnostic testing and ancillary services available to support neurology practice Imaging
• MRI available 4 or 5 non-holiday weekdays per week 7:30 am to 4:30 pm. MRI is
1.5T wide bore. MRA capabilities.
• Ultrasound available weekdays 8 am to 5 pm. Fluoroscopy (e.g., swallowing studies)
available most weekdays with speech pathologist and twice monthly with a
radiologist.
• CT and plain radiographs at all times. CTA capabilities.
• Nuclear medicine weekdays 8 am to 5 pm. If interested, might be able to develop service if operationally and financially feasible.
• During weekday business hours, images are interpreted by radiologist. Such
radiologist covers all 3 hospitals and is on site most Wednesdays and
selected Fridays.
• After-hours preliminary interpretations are provided by vRad for emergent and
urgent studies. Quality issolid. Routine studies are interpreted by radiologist
within 24 hours of test performed.
Neurodiagnostic
• EEG's are performed by technologist as needed, generally 3-5 days/week. Video
EEG's are not available locally.
• NCS/EMG's are performed as needed by Dr. and interpreted by him.
• Lumbar punctures are performed by Dr. on his patients or by CRNA's if so ordered.
Cardiology diagnostics
• Readily available during weekdays.
• Broad array of various types of stress tests, echo with or without contrast, TEE, TEE cardioversion, and monitor (Zio patch) testing.
Pulmonary diagnostics
• Readily available during weekdays.
• Broad array of PFT's, ABG, 6-minute walks, and overnight oximetry testing.
Sleep diagnostics.
• Home sleep studies available; interpreted by AVH sleep medicine physician.
• Attended sleep studies performed 3 nights/week with an occasional day
(nap) study if needed; interpreted by sleep medicine physician.
Rehabilitation Services
• Extensive services available for physical, occupational, and speech therapy.
• Special clinical interests/expertise among selected therapists related to neurology
include:
o Speech Therapy:
- Evaluate and treat patients with disorders of cognition, communication and
swallowing. Diagnoses include, but are not limited to, dementia, traumatic brain injury (TBI), Parkinson's disease, stroke, concussion, and other head injuries.
- Swallow evaluations performed include bedside swallowing evaluation,
modified barium swallowing study (MBSS), and fiberoptic endoscopic evaluation of swallowing (FEES).
- Certified to perform the LOUD portion of the LSVT BIG and LOUD program.
• Physical Therapy:

- Evaluate and treat patients with balance deficits, vestibular dysfunction post-
concussion syndrome, vertigo. Diagnoses include, but are not limited to,

Parkinson's disease, stroke, concussions, headaches, and TBI.
- Certified to perform the BIG portion of the LSVT BIG and LOUD program.

- Use Dividat Senso, an interactive testing and training device for cognitive- motor functions, with focus on balance, lower extremity coordination, and core strength.
• Occupational Therapy:
- Evaluate and treat patients with nerve compression, nerve entrapment, and
repetitive motion injuries. Diagnoses include, but are not limited to, carpal tunnel syndrome, cubital tunnel syndrome, radial tunnel syndrome, pronator teres syndrome, and thoracic outlet syndrome.
- Use prefabricated and custom splinting for the upper extremity.
