AgeRight Clinical Services was created to meet the ever-increasing medical needs of Post-Acute and Long-Term patients in Skilled Nursing Facilities. By providing patients with fast and reliable access to our dedicated group of clinicians, we have revolutionized the way in which healthcare is delivered in the Long-Term Care setting.

We are currently seeking experienced physicians looking to join our group of diverse, compassionate providers to help deliver the highest quality of care to our patient population.

Physicians with AgeRight Clinical Service will serve as Medical Director at various facilities throughout the Portland Metro area. In this role you will be responsible for rounding on new admissions at assigned facilities, focusing heavily on transition management, quality improvement, and reduction in hospital readmissions. Each facility also has the support of an Advanced Practice Provider and administrative team to support a healthy work life balance.

This is a fantastic opportunity for an experienced Geriatric, Family, or Internal Medicine Physician who desires to work in a facility-based environment, enjoys autonomy, and is looking to exercise their clinical skillset in our ever-evolving environment. You will have the opportunity to build relationships with your patients and make a meaningful difference in their lives.

Primary Responsibilities:

  • Plan, direct, and provide primary care and post hospital medical care to facility residents.
  • Collaborate and coordinate patient care with facility staff and APP.
  • Participate in Quality Improvement initiatives.
  • Timely completion of documentation.
  • Some PA supervision may be required.
  • Remain available to facility staff by phone and fax M-F, 8am-5pm.

Qualifications

    • Current, unrestricted Oregon Medical License.
    • Active DEA license.
    • Board certified in Geriatrics, Family Medicine, or Internal Medicine.
