Primary Responsibilities:

• Support Boulder Community Health's Discharge Planners in achieving effective care management throughout a patient's healthcare journey.
• Collaborate alongside our Nurse and Social Work team to provide post-acute medical and psychosocial resources to our patient population
• Assist in coordinating transitions of care from the inpatient to ambulatory settings

Required Qualifications:

• Pursuing a Master's of Social Work degree from an accredited college / university
• Actively enrolled in an accredited college / university during the duration of the internship. Internships are not intended for graduating students
• Must be eligible to work in the U.S. without company sponsorship, now or in the future, for employment-based work authorization (F-1 students with practical training and candidates requiring H-1Bs, TNs, etc. will not be considered)
• You will be asked to perform this role in an office setting, however, may be required to work from home temporarily due to space limitations

Colorado Residents Only: The hourly range for this role is $16.00 to $20.05 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
