Job Description

PURPOSE OF POSITION:

The Emergency Pharmacist is responsible for providing comprehensive clinical pharmacy services for the ED and all associated areas (e.g., pediatrics, trauma, urgent care), including pharmacokinetic and therapeutic consultations. In addition to performing the duties at a more difficult and responsible level, the following are typical tasks performed

DUTIES & RESPONSIBILITIES:
1. Participate in direct patient care in the Emergency Department
2. Comprehensive acute management of patients in the Emergency Department. Familiarity with the standards of patient care; understand the medical conditions, treatment goals, treatment drugs and protocols for Ambulatory Care Sensitive Conditions (ACSC: Asthma, COPD, Diabetes, CHF, Hypertension, Pneumonia, Dehydration and UTIs) and other patients that are referred.
3. Provide clinical support for the multidisciplinary healthcare teams for all Emergency Department patients including participating in acute care resuscitations, and attending code and trauma activations.
4. Participate in drug therapy management during emergencies including the preparation of acute critical medications.
5. Review of medication profiles to assure optimal pharmacotherapy treatment plans
6. Optimizing medication therapies to prevent and reduce medication errors
7. Therapeutic drug monitoring for appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication and drug interactions
8. Provide pharmacokinetic consultations when requested
9. Conduct patient education and counseling on medications.
10. Facilitate transitions of care for patients upon admission to ED, transfer to inpatient units and/or discharge.
11. Support/assist patient or caregiver to obtain prescriptions upon discharge; confirm prescription insurance coverage, formulary status and ability of patient to obtain medications.
12. Work with the Outpatient Pharmacy Department (OPD) for uninsured patients as needed.
13. Promote the use of the formulary by converting non formulary orders to formulary, when possible.
14. Provide patients/caregivers with discharge counseling as applicable.
15. Provide medication information to interdisciplinary healthcare team
16. Develop medication guidelines, order sets, and protocols in collaboration with clinical staff
17. Pursue opportunities for optimization of Electronic Medical Record related to patient care in the Emergency Department.
18. Document pharmacotherapy interventions within the Electronic Medical Record.
19. Collect, analyze and maintain all data for all documented interventions.
20. Facilitate, develop, and implement lectures to pharmacy staff in Emergency Medicine related topic.
21. Provide in-services to medical staff on pharmacological topics.
22. Provide in-services to nursing staff on pertinent pharmacy-nursing related topics
23. Coordinate policies and procedures for clinical activities (i.e. therapeutic interchange, unacceptable abbreviations, look alike-sound alike drugs, medication events reporting etc.)
24. Participate in quality improvement activities of the department.
25. Precept and mentor pharmacy students.
26. Participate in multi-disciplinary committees to support the appropriate use of medication therapy; conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization
27. Additional duties as assigned

Minimum Qualifications

For Level I:
1. Possession of a valid NY State license to practice pharmacy.

For Level II: In addition to meeting the qualification requirements of the lower level:
1. Two years of post licensure experience; or
2. Completion of a post graduate specialty residency or fellowship program; or
3. Current Pharmacy Board Certification.

For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:
1. One year of post licensure supervisory experience or specialty clinical experience.
