Description

As the Healthy Hub coordinator, this registered nurse will provide clinical expertise to the Healthy Hub Initiative and its clients. Performs community health screening. Reviews health risk assessment information and recommends risk reducing programs for our community. Provides clinical expertise to the Health Check program.

Qualifications:

Licenses & Certifications: Registered Nurse - RN
Basic Life Support - BLS
Extensive knowledge base in cardiovascular/ diabetes health. Demonstrated ability to teach, both one-on-one, and to large audiences. Exceptional communication skills, both written and verbal.
Self motivated and team oriented.
Minimum Required Experience: 5 yearDegrees: Bachelors degree in Nursing - BSN
Licenses & Certifications: Registered Nurse - RN
