  • Position Summary
    • The Transplant Coordinator is a critical member of the multidisciplinary team for the advanced heart failure patient population. The Transplant Coordinator is responsible for caring for the patients with end-stage heart failure during the evaluation, transplant listing and post-transplant phase. The coordinator is a point of contact for patient, family/caregivers and liaises with the multidisciplinary team to deliver the most efficient care. The coordinator must be proficient with UNOS allocation. The Transplant coordinator directly interfaces with the Brigham and Women's (BWH), Tuft's Medical Center, Mass General Hospital (MGH) Yale New Haven & Virginia VA Cardiac Transplant Programs and follows their specific center protocols. The responsibility includes significant collaboration with physicians, cardiac departments, outside medical facilities, patients and their families and other support services. The Transplant Coordinator is the "face" of the Program and should have the skills to reflect the program in a positive manner and apply advanced patient assessment skills and knowledge in the area of immunology and tissue typing to perform necessary functions of the job.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: Graduate of an accredited nursing program.
    • License/Certifications: Current License to practice as a Registered Nurse in the State of Maine; BLS/ACLS certified.
    • Experience: Minimum 5 years of cardiovascular nursing experience with 2 years of critical care experience preferred.
    • Excellent interpersonal and communication skills to effectively interact with applicants, accepted patients, families of patients, and all levels of healthcare providers. Ability to develop collaborative working relations with multiple disciplines, departments and outside healthcare organizations.
    • Thorough and significant knowledge of the theories, practices, processes and results of current and emerging advanced heart failure technologies and treatment regimens.
    • Proven successful ability to manage and coordinate complex projects and processes from inception to implementation, and follow up.
