Job Description

This position reports to the HIV Clinical Quality Director, HIV Services (HIVS) for the Office of Ambulatory Care & Population Health. The position involves working with clinical and administrative leaders across NYC H+H to develop and establish a sustainable and equitable Hepatitis C Screening and Care program, with the goal of increasing access, from testing through to cure. NYC H+H provides care for thousands of individuals with Hepatitis C and is a key component of effort to eliminate Hepatitis C in NYS and NYC; the Program Manager position is central to NYC H+H's Hep C response. This is a grant funded and will be directly responsible for all reporting and grant deliverables required by the sponsor, in addition to all Hepatitis C data reporting request from other funding sources (e.g. City Council).

Position responsibilities include: ability to develop a detailed understanding of facility operations related to Hepatitis C care across H+H; coordinating and supporting the efforts of facility-based Hep C Navigators; developing standard workflows; supporting the development of electronic medical record (EMR) clinical decision support tools and data resources; creating and providing training on support tools to staff of a variety of positions; high quality written and verbal communication and relationship building skills; collaborating with clinical and administrative leads to identify, design and implement quality improvement initiatives; and collaborate on publications and research. We are looking for a highly motivated, self-starter, with excellent problem-solving skills who has experience working in outpatient clinical environments and is dedicated to improving healthcare access and quality for all New Yorkers.

GENERAL TASKS AND RESPONSIBILITIES WILL INCLUDE:
  • Work collaboratively with clinical and administrative leaders, mid-level providers and Patient Navigators to integrate Hepatitis C navigation with the goal of increasing screening and cure rates.
  • To best integrate the Hepatitis C care navigation program, an ability to learn and rapidly develop a strong understanding of the specific processes of the targeted facility/clinic is necessary. This includes collaborating and developing relationships with existing staff, reviewing operational processes (care team staffing models, clinical flow processes, clinic design, scheduling and templates, visit types, front desk processes, referral tracking, missed visit follow up); knowing clinical standards (patient screenings, results follow up, referral management, patient satisfaction); and maximizing EMR processes (clinic and billing documentation, referral tracking, QI and grant reporting)
  • Develop clinical workflows and provide training on clinical workflows related to direct patient outreach and case finding, screening and diagnosis, linkage to care, support to start treatment and achieve cure of Hepatitis C.
  • Develop and implement evaluation tools to track progress toward meeting program goals.
  • Complete all required data collection, reporting and other grant deliverables.
  • Complete all required data collection, reporting and other deliverables to City Council related to Hepatitis C funding.
  • Establish working groups to address specific clinical or procedural concerns to ensure systems are reflective of the needs of patients and staff.
  • Support and oversee the work of facility-based Patient Navigators.
  • Provide direct support and advice to facilities/clinics on their Hepatitis C efforts.
  • Participate in practice improvement meetings/sessions with facility teams; including directly providing expertise, coaching, and guidance as needed.
  • Periodically assess performance and progress and seek solutions to overcome obstacles by addressing concerns in an effective manner.
  • Work with HIVS to strategize and mobilize resources related to operational processes, clinical standards and workflows, and EMR processes and supports.
  • High quality written and verbal communication including narrative components of grant reporting, presentation and training development and professional email communication.
  • Ability to rapidly develop understanding of data sets related to Hepatitis C in order to communicate essential points to front line staff.
  • Create feedback mechanisms to support sustainability of the program and ensure facility/clinic administrative and clinical leaders and Patient navigators are kept informed and have their concerns addressed.
  • Strategize with clinics caring for a disproportionate volume of patients with Hepatitis C to establish patient-centered initiatives resulting in higher cured rates. This includes clinics that support individuals with substance use disorder, individuals involved in the justice system, and people experiencing homelessness.
  • Other related duties, as assigned.


Minimum Qualifications

Educational and/or experience requirements will vary requiring diverse kinds and levels depending on the content, complexity and variety of project. Compensation levels will correspond to the requirements developed for the designated project.

In no event will the acceptable minimum be below:
1. A Baccalaureate Degree from an accredited college or university in a specialization discipline consistent with project content; and,
2. Four years of substitutable and relevant health care experience related to the objectives of the project; or,
3. A satisfactory equivalent combination of experience, training and education.

Department Preferences

  • At least five years of experience in healthcare with ambulatory care practice experience preferred.
  • Relevant experience in program implementation, program management, clinical supervision, quality improvement, or research related to clinical practices preferred
  • This position relies heavily on critical thinking, strong interpersonal, and communication skills, both in-person communication and remote.

KNOWLEDGEABLE IN:
  • Knowledge and experience of clinical care and ambulatory practice environments.
  • Knowledge and experience related to multi-site project management.
  • Knowledge and experience related to organizational change in healthcare and/or implementing process improvement projects.
  • Knowledge of Hepatitis C clinical care and efforts to eliminate Hepatitis C.
  • Knowledge of quality improvement processes and data tools, as well as a basic understanding of programmatic research and data.
