UPMC Home Care Aide Somerset

UPMC is hiring a full-time Home Care Aide for the Somerset area. You will work Monday - Friday daylight shifts from 8:00 a.m. - 4:30 p.m.
HCA will perform daily patient visits, collaborate with team members, notify staff of patient concerns or changes, and perform personal care.

Full-time benefits, and mileage reimbursement, flexibility with scheduling. Weekend rotations.

Responsibilities:
  • Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Proven ability to facilitate change.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Participates in all required agency meetings.
  • Must maintain annual competency evaluation scores as determined by regulatory requirements

Qualifications:

  • High School Diploma or GED - Required
  • One (1) year experience - Preferred
  • Certified Nurse Aide -Preferred
  • Minimum of 75 hours of Nurse Aide training- Required.
  • If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
    Licensure, Certifications, and Clearances:
    Preferred: Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. Required: CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
