The Home Care Coordinator/Discharge Planning (HCC/DCP) Manager is a healthcare professional who has a thorough understanding of available home care services and products, eligibility requirements, and resources available in the community and manages the functions within designated teams. The HCC DCP Manager acts, in a leadership capacity, as a discharge planning resource to Hospital staff involved in home care planning, and facilitates continuity of care for the patient referred to receive home care services.

Experience and qualifications:
  • Graduate of accredited RN program with B.S. in Nursing OR Bachelors in Social Work. if nursing, RN in the state of Maryland, and CPR
  • Two (2) years home care experience required with progressive management.
  • Demonstrated excellence in clinical assessment, home care planning and problem-solving skills.
  • Demonstrated clinical expertise in many clinical areas, i.e. oncology, infusion therapy, pediatrics.
  • Customer service.
  • Thorough knowledge of community resources including methods for reimbursement.
  • Thorough understanding of agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services.
  • Practical knowledge of management principles such as conflict resolution, delegation, and motivation.
