Position Overview:
  • The position acts as a discharge planning resource to Hospital staff involved in home care planning, and facilitates continuity of care for the patient transitioning to home based services. Serves as on-site home-based care expert but does not provide hands-on clinical care.

Location:
  • On the Johns Hopkins Hospital Campus.

Schedule:
  • Monday - Friday 8:30am-5:00pm

Pay Rate:
  • $42.00 -$46.00 per hour (Based on experience).

Requirements

  • Assesses patient's home care needs, advises team, and assists in developing a safe and timely discharge plan with discharge planning team.
  • Develops safe and timely referrals for home care services.
  • Assists in the intake functions and coordinates the plan of care for patients being referred to JH entity.
  • Responsive to the discharge planning needs of the hospital.
  • Provides referral sources with ongoing and current information about available home care services and protocols.

Working Conditions:
  • Work is generally performed in a Hospital Setting which includes entering patient rooms, working of nursing units, and significant walking between nursing units and buildings.
  • Possible exposure to communicable diseases, hazardous materials, and pharmacological agents with little likelihood of harm if established health precautions are followed.


Qualifications

  • MS Social Work from an accredited school of social work required.
  • Must be licensed by the Maryland Board of Social Work Examiners at the LMSW level.
  • AHA Healthcare Provider CPR required.
  • One (1) year acute care clinical or home care or home care discharge planning experience required.
  • Demonstrated excellence in transitions to home care and problem-solving skills.
  • Demonstrated experience in clinical settings.
