Community Care, Inc. is currently seeking a dependable home health aide to provide in home care to a 13 year old in the New Stanton area. Home health aide would be responsible for assistance with ADLs. Wonderful patient and family!

Shifts available:
  • Afterschool/evening hours 3 evenings/week - No weekends!! (16 hours/week)
  • Monday, Tuesday, and Thursday.

POSITION SUMMARY: The home health aide (HHA) is a paraprofessional staff member educated to provide personal care and to assist with activities of daily living, exercise programs, and related services in the home for the sick and disabled, when no family member can assume this responsibility. This employee functions under the direction, instruction and supervision of the registered nurse and/or therapist.

I. CLINICAL SKILLS
  1. Helps the patient with personal hygiene which includes but is not limited to; bathing, nail and skin care, oral hygiene, shampooing.
  2. Prepares nutritious meals or snacks for the patient, as assigned.
  3. Assists with treatments, such as simple dressings, range of motion, ambulation and routine exercises.
  4. Provides basic emotional support to the patient and other members of the household including dealing with difficult behaviors.
  5. Helps to maintain a clean, safe and healthy environment for the patient.
  6. Makes pertinent patient observations, recognizes emergencies, and seeks guidance appropriately.
  7. Reminds the patient to take medications according to plan of care. Does not administer medications.
  8. Works with personnel providing care from other community agencies or privately hired by the family, as directed by nurse or therapist.
  9. Recognizes and reports abuse or neglect.
  10. Escorts safely to medical appointments, social activities, shopping.­


II. DOCUMENTATION
  1. Accurately reads and follows the HHA Plan of Care.
  2. Accurately completes home health aide notes/flow sheet for each visit and submits it to the office within electronic medical record.

III. COMMUNICATION
  1. Reports any change in the patient's mental or physical condition or his home situation to the RN clinical supervisor and/or to the care manager for the patient.
  2. Calls RN clinical supervisor or designee with any immediate patient clinical needs.
  3. Addresses non-clinical issues with the scheduling coordinator.
  4. Maintains confidentiality of client information adhering to HIPAA regulations.
  5. Submits monthly availability for scheduling.

IV. PROFESSIONAL STANDARDS
  1. Complies with infection control and safety policy procedures.
  2. Adheres to all policies and practices of the agency to maintain established Standards of Care.
  3. Carries out only those activities for which s/he has been trained and supervised, bringing to the supervisor's attention any assignments that s/he does not feel competent to carry out.
  4. Maintains a professional standard of conduct, projecting a positive image of the agency at all times.
  5. Works with patients in a caring, therapeutic and professional manner.

V. PROFESSIONAL DEVELOPMENT
  1. Attends twelve hours of health-related continuing education per year.
  2. Attends annual mandatory in-service sessions.
  3. Participates in additional staff meetings, care conferences, educational sessions and the Performance Improvement Program, as appropriate.

VI. PERFORMS ADDITIONAL DUTIES AS ASSIGNED

VIl. QUALIFICATIONS
  1. Eighteen years of age or older. High school graduation or GED preferred.
  2. Successfully passes a competency evaluation of skills required for patient care. Completion of a formal home health aide/nursing assistant (CNA) program preferred.
  3. Scores at or above the 70% score on the HHA pre-employment test.
  4. One year of supervised employment as a HHA or nursing assistant preferred.
  5. Ability to understand and carry out directions and instructions and keep appropriate records.
  6. CPR certified.
  7. When required to travel from one assignment to another on the same day, a reliable, independent means of transportation is required. Drivers must have a valid driver's license and car insurance limits adhering to the limits established in the personnel policies.
  8. Obtains and retains a clear criminal history, Childline, FBI, OIG, Medicheck, EPLS and any additional required regulatory clearances.
