Job Summary

Join UT Southwestern as a Case Manager Registered Nurse (RN ) within our Home Health Department and help with our commitment to deliver compassionate care and all-around excellence. As a RN Case Manager, you will support the Home Health by providing excellent patient education for the patient and their families. The role will include comprehensive health and psycho-social evaluations, monitoring of the client's conditions, health promotion and prevention, coordination of services, and teaching and training activities.

Experience and Education

Graduate of accredited nursing program; BSN preferred. Current licensure as Register Nurse by Board of Nurse Examiners for the State of Texas (or BNE Compact State). Valid Texas driver's license, proof of driver's insurance and Basic Life Support (BLS) certification (accredited course by the American Heart Association (AHA) or American Red Cross (ARC) required. Two (2) years RN experience with one (1) year experience with adult IV expertise required. Other RN experience including high tech and home health expertise preferred. Basic computer skills and knowledge; prefer knowledge of InterQual and TMF care criteria.

Job Duties
  • Provides direct nursing care, including patient observation and delivery of prescribed treatments. Conducts initial and ongoing comprehensive assessments of home health patient needs, including OASIS assessments at appropriate time points.
  • Evaluates outcomes of patient care and identifies discharge planning needs. Consults with other disciplines as required. Adjusts nursing care processes as necessary.
  • Maintains patient records and charts to ensure accurate record of treatment, including med profile, care plan, communication notes, etc.
  • Provides instructions to patients/families for use of products and services utilizing nursing process to address learning needs.
  • Prepares, administers, monitors, and discontinues IV fluids. Accesses IV delivery system (ie. peripheral IV start, implant cath, broviac, etc.).
  • Administers medications and treatments as prescribed by the physician.
  • Performs phlebotomy, laboratory specimen collection and point of care testing.
  • Responsible for overall delivery of quality patient care that meets or exceeds established standards and provides assistance in activities to meet state and/or federal regulatory guidelines.
  • Maintains safe, comfortable, and therapeutic environment for patients/families in accordance with established standards.
  • Participates in Continuous Quality Improvement (CQI) Program.
  • Provides supervisory visits for Home Health Aides and LVNs as required by state and federal regulations.
  • Responsible for participating in on-call rotation and emergency call according to agency policy.

Working Conditions

Exposure to dust/dirt/fumes, exposure to disease, move and position patients, bending or reaching, driving a vehicle, more than average exposure to variable weather and road conditions, walking, annual physical examination, lifting over 50 pounds, available to work all shifts.
