Responsibilities

Dignity Health's home health services provide comprehensive and compassionate care for patients who have recently been discharged from the hospital, who need short-term skilled nursing, physical therapy, or speech therapy. The Home Health RN provides highly skilled nursing care in the home setting as the admitting nurse providing services such as wound care IV medication administration in addition to supervising LVN care delivery and care coordination with the multidisciplinary team. You are the front line clinician communicating with the physician overseeing the plan of care. As a the RN, your role is integral to leading the case conference and improving clinical outcomes.

Qualifications

Minimum:
  • 1 year home health nursing experience in the past 5 years.
    Exception: Will consider an applicant with 1 year acute care Med/Surg, Tele, ICU Nursing experience in the last 5 years only when adequate staffing allows.
  • Currently licensed as an R.N. in California.
  • Current driver's license. If out of state license, consistent with California DMV regulations, must obtain California driver's license within the first 30 days of employment. Possession of automobile insurance, and access to an operating motor vehicle. Current BLS card.
  • BLS certification (AHA endorsed)

Bachelors Degree in Nursing

Public Health certificate
