Responsibilities:

Provides direction and supervision to Hospice Care Planners. Plans, coordinates manages and evaluates the intake coordination process.home care, nursing, RN, homecare, nurse, home care nurse, home care RN

Qualifications:

RN, BSN required, Masters preferred. Four-five years in a Certified Home Health Agency or Long Term care program. Good Communication and Leadership skills required. Excellent Customer service skills also required. Ability to plan, develop and expand the program within the larger health system.
