Job Description:
PURPOSE OF JOB:
To assure the delivery of spiritual services to the patients and families enrolled in the Hospice program, consistent with the patient/family belief system and with the hospice policies and procedures.
II. ESSENTIAL FUNCTIONS:
A. Assesses, plans, implements and coordinates all spiritual aspects of patient care. Spiritual assessment of
patient/family coping, strengths and weaknesses and counseling related to the same, and bereavement risks.
Works with grieving families and is comfortable dealing continuously with death and dying. (25%)
B. Communication and collaboration with patients, families, healthcare providers, and community resources in order to achieve patient/family, and program goals and positive health outcomes. Establish goal-directed plans that reflect own observations as well as those shared by other Hospice team members. Understands and is able to make appropriate community referrals. Documentation is accurate and on time 90%. (25%)
C Participates in other Hospice functions and activities, such as volunteer training, community speaking engagements,
memorial services, fundraisers, and performance improvement activities 3-4 times a year 100%. (25%)
D. Demonstrates fiscal responsibility through appropriate assessments and interventions related to patient financial situation, and
appropriate referrals, as well as demonstrating good time management and productivity. Promotes patient safety through
adherence to protocols, procedures, guidelines, regulations, standards, and safety goals. Maintains clinical competency,
documentation standards, and established timelines. Follows policies 100%. (25%)

Job Qualifications:
QUALIFICATIONS:
- Education: Masters Degree in Divinity or Masters Degree in Religious Arts from an accredited seminary, university, or divinity program..
- Experience: Ministry in local church or pastoral/clinical experience of at least 2 years highly preferred, or
- Other Skills: Must be able to deal and communicate effectively with a wide range of people, work with all faith groups. Comfortable with public speaking situations. Must drive a car in inclement weather.
- Training time required once hired: six months
