Hospice Nurse Practitioner NP

PathWell Health is a home health and hospice service provider in northern Virginia. We are seeking a dedicated and experienced Hospice Nurse Practitioner to join our team on a part-time, hourly basis. The ideal candidate will bring a strong background in hospice care, demonstrating expertise in participating in the Interdisciplinary Team (IDT), collaborating with medical professionals, and ensuring optimal patient care.

Responsibilities:

• Order prescriptions as needed in consultation with the medical doctor, ensuring timely and accurate administration of medications.
• Conduct Face-to-Face assessments if and when required, maintaining compliance with regulatory requirements
• Actively engage in IDT meetings to contribute valuable insights and collaborate with interdisciplinary team members.
• Demonstrate responsiveness and maintain on-call availability to order medications and address urgent patient needs promptly.
• Liaise with our medical doctor and registered nurses to coordinate and optimize patient care plans.

Qualifications:

• Current Nurse Practitioner license in the state of VA
• Ability to work collaboratively within an interdisciplinary team.
• Experience in hospice and palliative care.
• Experience in ordering prescriptions and managing medication
• Demonstrated ability to conduct Face-to-Face assessments
• Commitment to maintaining the highest standards of patient care.
