Are you highly organized and detail-oriented, passionate about supporting others? We're seeking dedicated caregivers to join our team and play a vital role in providing in-home care for our clients. You'll be responsible for following personalized care plans, ensuring each client receives the specific assistance they need. This may encompass companionship, meal preparation, light housekeeping, errands, transportation, and even personal care services like bathing and dressing. A minimum of 1,000 hours of experience working with geriatric or chronically ill individuals is preferred (with some exceptions as outlined below). The ideal candidate possesses excellent communication skills, a commitment to exceptional care, and a desire to learn and grow within the caregiving field!

Minimum Qualifications:
  • A minimum of 1,000 hours of experience working with geriatric or chronically ill individuals is preferred (approximately 6 months full-time or 12 months part-time). Exceptions are made for recently licensed CNAs or HHAs, or for those providing companionship and housekeeping services only.
  • A CNA or HHA license is a plus, but not mandatory.
  • Current TB testing is required.
  • You must be physically able to perform the duties of this position, including safe transfer techniques for assisting clients.
  • Reliable transportation with a valid driver's license and car insurance is required if transporting clients.
  • Excellent communication skills are essential for effectively interacting with clients, care managers, and other team members.
  • A mature, caring, punctual, and reliable demeanor is key to building trust with clients.
  • You must be able to pass a criminal background check (national and any required by state).
  • Verifiable professional and personal references are required.

Essential Duties and Responsibilities:
  • Understanding and Implementing Care Plans: You'll meticulously review and comprehend each client's care plan, ensuring you provide the necessary assistance as outlined.
  • Individualized Client Support: Your duties will vary based on client needs but may include companionship, preparing and serving meals, light housekeeping, transportation, shopping, medication reminders, bathing, body care, dressing, feeding, ambulation, positioning, safe client transfers, toileting, and providing protective supervision.
  • Client Communication and Advocacy: You'll be responsible for notifying the Care Manager of any client concerns, changes in care or client condition, and client emergencies. This ensures timely intervention and a high level of care.
  • Safety and Compliance: Following company Infection Control practices is essential. You'll also need to notify the office if you require Personal Protective Equipment (PPE) or additional supplies to perform your duties safely.
  • Timekeeping and Documentation: ClearCare Telephony Instructions will guide you in clocking in and out for all scheduled shifts. You'll also need to accurately update your task status for each client before leaving their home. For non-client-specific hours (like training time), you'll submit a Time Log and Time Certification for Non-Visit Time form.
  • Professional Appearance and Conduct: Maintaining a professional demeanor and adhering to the company dress code as outlined in the Employee Handbook is expected.
  • Dependability and Teamwork: Reporting to work on time and ready to work is crucial. You'll also participate in ongoing training to stay updated on client care needs and company policies. Regular staff meetings are essential for collaboration and information sharing. Finally, adhering to all company policies and procedures as detailed in the Employee Handbook ensures a smooth and efficient work environment.

Organizational Relationship:

The caregiver reports directly to the Care Manager for guidance on client care and receives scheduling direction from the Staffing Coordinator.

Physical and Environmental Demands:

The role requires physical ability to safely reposition and transfer clients using proper techniques and equipment. Travel to client residences using reliable transportation is also necessary.
