MAJOR FUNCTION:

The Mobile Outreach Coordinator provides outreach education, enrollment and social service case management for the Indigent Health Care Program (IHC), intersecting with low income and indigent populations of Polk County for service connectivity. Assist residents with initial and re-enrollment health plan applications through field, office or home interviews. Responsible for driving mobile unit to pre-determined sites for scheduled outreach efforts, including health fairs and community events, to promote IHC Programs. The Mobile Outreach Coordinator will be a team player in supporting community partners, who are utilizing the mobile unit, and managing patient flow, when necessary. Reports directly to the Community Health Planning Manager.

ILLUSTRATIVE DUTIES:

The Mobile Outreach Coordinator is responsible for safely operating the mobile unit for outreach activities to promote IHC Programs, as directed by the Community Outreach Supervisor. Adhere to the Outreach Plan initiatives and provide education and information on services and programs offered to County residents. Interview residents for assessment of need for multiple program/services and community resources. Evaluates financial, social, environmental, medical, family status, and related information, making proper disposition or referral of case on basis of eligibility. Gathers, reviews, and uploads applicant's documentation into the County's Shared Data Information System to determine eligibility. Prepares and completes case reports and correspondence, applicable and required forms and statistical reports. Provide applicant information on services and programs offered. Acts in a liaison capacity with medical personnel and other social service agencies on the applicant's behalf. Ensures confidentiality of applicant's information in compliance with existing county, state and federal laws, including the Health Insurance Portability and Accountability Act (HIPAA) privacy rule and regulations.

Perform a variety of support tasks relating to outreach, including organizing outreach supplies and equipment and assisting in maintaining outreach reports and statistics. Required to set up and break down site equipment and structures related to the mobile outreach events. Completes daily inspection of mobile unit's exterior/interior and equipment and maintain logs of inspection. Keeps the mobile unit clean and free of debris. Immediately reports to supervisor any operating problems, malfunctions, or incidents out of the ordinary routine and coordinates and schedules routine and episodic maintenance.

Assist in special projects and perform other duties as assigned. May be required to work occasional evenings and weekends.

Working knowledge of Federal, State, County and community social service programs including eligibility requirements, application procedures, and other relevant program policies.

Ability to become familiar with other social services agencies and resources.

Ability to correctly apply applicable laws, regulations, and policies to individual cases.

Skill in interviewing clients to identify deficiencies in personal, legal, social, economic, and health needs.

Ability to perform independently or as a member of multiple team efforts across diverse groups having responsibilities for providing County services.

Skillful writing, reporting, and record keeping abilities.

Ability to communicate, both orally and in writing.

Knowledge of computer and standard office equipment. Ability to learn software system utilized by the Polk HealthCare Plan.

Ability to collect, organize and evaluate data and to develop logical conclusions.

Proficient in Microsoft software applications.

May be required to lift, move or maneuver equipment and/or supplies that weigh up to thirty (30) pounds

MINIMUM QUALIFICATIONS

Graduate of an accredited two (2) year college with major course work in Social Work, Sociology, Psychology or a related field and have a minimum of two (2) years social service interviewing, counseling or related case management experience.

Must possess a valid Florida CDL at the time of employment or obtain valid Florida CDL within 6 months to 1 year of employment. Clean record in accordance with the Safe Driving Policy.

4 years of Social Services training and experience may be substituted for the minimum qualifications, with exception of the driving record and intent to obtain CDL within the required timeframe.

SPECIAL REOUIREMENTS

This position may be required to report for work when a declaration of emergency has been declared in Polk County.

Must undergo a Level 2 (FDLE) screening submitted electronically before commencing work. Those obtaining this certification must maintain certification as a condition of employment.
