Responsibilities

GENERAL PURPOSE:

Provides professional comprehensive care aligned with ICARE values to patients of all the Strong Memorial Hospital/ Strong Ties Assertive Community Treatment (ACT) teams. Collaborates with health, behavioral health and social service providers and is responsible for assessing patient's needs, developing, and managing care plans with patients enrolled in ACT services.

The IPS Employment specialist will work with all ACT teams and their patients to provide the necessary assessment and in community support to assist patients in engaging in competitive, integrated employment.

RESPONSIBILITIES:
  • Conducts assessments for ACT enrollees identifying service needs that contribute to developing patient centered care plan goals around engaging in competitive, integrated employment.
  • Carries out strategies for implementation of treatment plan by: establishing contact with various service providers and family members; assuring appropriate utilization of life support services in the community; and takes initiative for suggesting appropriate community services that would enable the patient to function at his/her optimal level in the community.
  • Under general direction and with considerable independence, works with all Strong Ties ACT teams and their patients around treatment plan issues on competitive employment. Provides services in the community with patients for at least 65% of their FTE.
  • The IPS Employment Specialist has an active role in service plan reviews and is a regular attendee on team meetings/case conferences to coordinate efforts with vocational staff of the ACT teams.
  • Documents patient contacts and patient progress in the medical record and assists patients with paperwork, updating treatment plans around employment goals, correspondence, etc. necessary to ensure linkage to appropriate resources.
  • Documents patient outreach and services in electronic medical record system and ensures that all documentation is timely, accurate and in compliance with Hospital policies.
  • Establishes and maintains cooperative working relationships with community providers to establish job development and creating a network of employers and job opportunities for patients enrolled in ACT.
QUALIFICATIONS:
  • Bachelor's Degree in an appropriate human services field and; or an equivalent combination of education and experience required.
  • 2-3 years of experience providing direct services to individuals diagnosed with serious mental illness, intellectual/developmental disabilities, alcoholism/substance abuse required; or experience effectively linking individuals with services that address social determinants of health required.

Pay Range

Pay Range: $52,187 - $73,050 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
