Intake Coordinator (Bilingual- Spanish speaking)

Looking for an Intake specialist who will be responsible for obtaining the necessary information from any/all potential patients in order to initiate the enrollment process to enable the patient to receive long-term home care services or Consumer Director Personal Assistance Services (CDPAS).

The Intake specialist is also responsible for data entry of all of the collected information. The Intake specialist ensures the insurance eligibility of all patients and the Department of Health requirments for Personal Assistants (PA'S) that consumers select to provide them with homecare services. The Intake specialists follows up with the insurance provider for all authorization and eligibility concerns. He/she also follows up with any/all potential patients/consumers/ PA's to ensure the timely collection of all data that is required to complete the enrollment process.
