Essential Functions of Intellectual Disability Case Manager:
  • Supervises and directs subordinate employees in their work, in the interest of the employer, using independent judgment and discretion, regarding duties. Initiates and executes constructive discipline and performance evaluations in accordance with Human Resource policies and procedures
  • Participates in the facility Quality Assurance process. Serves on committees and develops/implements improvement.
  • Maintains and implements department policies and procedures; ensures staff compliance.
  • Supports a positive and professional image through leadership activities that emulate the organization's mission statement and encourage a spirit of cooperation and teamwork.
  • Completes incident reports as instructed.
  • Develops Behavior Intervention Plans and revises as needed.
  • Identifies, reports, and responds to extreme behavior, such as verbal aggression, biting, hitting, kicking, throwing items (food, objects, bodily fluids), self-inflicted injuries, property damage, etc., in accordance with applicable local, state and federal regulations and Lake Drive policies and procedures.
  • Available for client emergencies 24 hours a day, 7 days a week.
  • Reviews and participates in appropriateness of referrals to facility, including conducting on-site evaluations when necessar
  • Participates in Human Rights Committee process ensuring that client rights are practiced
  • Completes financial transactions for the purpose of money management within the established guidelines for the individuals.
  • Assists clients with financial/budget needs or concerns.
  • Assists Business Office as requested with Social Security and Medicaid reviews as needed to aid in keeping benefits active
  • Works with the Business Office to ensure that individuals spend the minimum required Social Security funds per established guidelines
  • Participates in and/or ensures completion of monthly Resident Council Meetings
  • Ensures monthly activity calendar is competed based on client requests.
  • May participate and instruct individuals and staff in recreation and leisure activities using necessary recreation and leisure equipment.
  • Always encourages community integration and inclusion.
  • Other duties as assigned

Qualifications of Intellectual Disability Case Manager:
  • Bachelor's degree in related area of human services is preferred
  • One year of experience in the field of developmental disabilities is preferred
  • Candidates must maintain a working email address and phone number for employer communication.
  • Attends mandatory in-services and facility/department meetings as required.
  • Must be able to give verbal instructions to the individuals and staff as well as write, type, interpret and document Program Plans
  • Valid (state) Driver's license.
  • Willing to obtain and maintain MANDT instructor certification.
  • Attends mandatory in-services and facility/department meetings as required
  • Must be able to bend, stoop, run and walk for periods of time.
  • Must be able to utilize proper body mechanics while safely assisting with independent/dependent transfers and positioning.
