Internal Case Manager | RN or LPN

BaneCare Management is looking for a new Internal Case Manager (RN or LPN) to join our Clinical Reimbursement Team! BaneCare proudly oversees 10 Skilled Nursing & Rehabilitation Centers across the Massachusetts area in addition to 2 Assisted Living Facilities. The Internal Case Manager is responsible to ensure the collaborative process of assessment, planning, facilitation and advocacy for options and services to meet the patient's health needs through communication and available resources to promote quality and cost-effective outcomes.

JOB TYPE:

• Hybrid position - Remote with some in person/on-site work when designated/needed (40 hours/week; Full Time)

Job Duties for Internal Case Manager:

• Oversee the managed care census for assigned homes (will have multiple nursing homes to cover)
• Ensure timely updates/communications with both the managed care companies and centers.
• Monitor residents approved levels for changes in condition and communicate when higher levels may be needed.
• Monitor for carve outs- high-cost meds - work with managed care companies to obtain carve outs as needed, when changes occur.
• Excellent origination skills to keep track of managed care census for assigned homes.
• Position will report to SR Director of Reimbursement / VP of Reimbursement

Qualifications:

• Graduate of an approved school of Nursing with a license as a Registered Nurse/Licensed Practical Nurse.
• 1+ years of Case Management or MDS experience needed
• Able to read, write, and communicate effectively in English.
• Experience in case management which includes service to short/long term facility-based residents preferred.
