Title : Field Case Manager



LOCATION : Braintree, MA / Quincy, MA



Shift: Day



Client : American AFC



Rate Range : $19/Hr - $21/Hr on W2 or $23 -$25/hr on 1099

Home Visit : Min 40 visit / week.

Overview: We are seeking a dedicated Field Assessment Healthcare Case Manager to join our dynamic team. In this role, you will be responsible for conducting thorough assessments of healthcare cases in the field, ensuring that the needs of clients are met with the highest standards of care.



Key Responsibilities:

• Conduct comprehensive assessments of healthcare cases in various field settings.
• Collaborate with healthcare professionals, clients, and their families to develop personalized care plans.
• Coordinate and oversee the implementation of care plans, ensuring adherence to established protocols and standards.
• Provide ongoing support and guidance to clients and their families throughout the care process.
• Maintain accurate documentation of assessments, interventions, and outcomes.

Qualifications:

• Bachelor's degree in nursing, social work, or a related field (Master's degree preferred).
• Previous experience in case management, preferably in a healthcare setting.
• Strong interpersonal and communication skills.
• Ability to work independently and collaboratively in a fast-paced environment.
• Proficiency in Microsoft Office suite and electronic medical record systems.
