JHPC PERSONALIZED CARE SPECIALIST

JHCP Personalized Care Specialist Position Summary:

Johns Hopkins Personalized Care is a subscription-based concierge medicine practice. This position is responsible for coordinating the care of patients, including, but not limited to, scheduling, registration, follow up care, prescription refills and responding to member requests for information. Emphasis will be placed on the scheduling and registration of patient appointments including gathering all necessary patient information, updating EPIC, patient education and answering patient requests for information. This position will manage relationships with patients and/or their designee and provide direct support for the Physicians and Advanced Practice Providers at the practices. This position may also be responsible for billing services provided on behalf of patients enrolled in the Personalized Care practices

JHCP Personalized Care Specialist Key Accountabilities:

• Coordinates medical care for patients subscribing to the Johns Hopkins Personalized Care practice; Assists patients during the scheduling process and in-person appointments.
• Communicates patient needs on behalf of the patient to Physicians and/or APPs. Acts as a liaison within the practice to Johns Hopkins Medicine,
• Interacts with patients, practice and JHM employees, referral sources, payers, etc.
• Gathers, compiles and forwards medical records to applicable parties.
• Engages in analysis and problem solving of unique situations, as well as routine patient and patient flow issues.
• Implement best practices when handling patients and inquiries
• Accurately and efficiently uses EPIC practice management system for scheduling, patient data, and internal practice communications
• Performs basic office organization functions including but not limited to inventory management, ordering, and invoice processing

Work Schedule: FT, Day shift, 40 hrs/wk
Work Location: Lutherville, MD

JHCP Personalized Care Specialist Key Qualifications:

• BA or BS required, or equivalent work experience
• Advanced proficiency in oral and written English.
• Excellent knowledge of customer service best practices and concierge services catered to opinion leaders
